Indications

  • Treatment of hypertension

Pharmacology

  • Block vasoconstrictor and aldosterone-secreting effects of angiotensin-II
  • Thiazide diuretic affecting renal tubular mechanisms
  • Reduction of plasma volume and increase in renin activity
  • Angiotensin-II receptor antagonist linked to potassium loss reduction

Dosage & Administration

  • Usual starting dose 20/12.5 mg once daily
  • Individualized dosing
  • Titration based on blood pressure response

Interaction

  • Olmesartan has no significant drug interactions reported
  • Hydrochlorothiazide may interact with alcohol, barbiturates, narcotics, antidiabetic drugs, other antihypertensive drugs, corticosteroids, ACTH, and lithium

Contraindications

  • Hypersensitivity to any component of the product
  • Anuria or hypersensitivity to sulfonamide-derived drugs

Precautions & Warnings

  • Periodic determination of serum electrolytes
  • Risk of hypokalemia, hyponatremia, and hypochloremic alkalosis
  • Potential hyperuricemia in certain patients
  • Impaired renal function

Use in Special Populations

  • Usual regimens may be followed for renal impairment patients with creatinine clearance >30 ml/min
  • Loop diuretics preferred to thiazides in severe renal impairment
  • No dosage adjustment necessary with hepatic impairment
  • Safety and effectiveness not established in pediatric patients
  • Caution in dose selection for elderly patients

Overdose Effects

  • Manifestation of hypotension and tachycardia in Olmesartan overdose
  • Signs and symptoms caused by electrolyte depletion in Hydrochlorothiazide overdose
  • Risk of accentuated cardiac arrhythmias if digitalis also administered
