Pharmacology

  • Imipenem, a penem antibacterial
  • Cilastatin, a renal dehydropeptidase inhibitor
  • Bactericidal activity from inhibition of cell wall synthesis
  • Affinity for penicillin binding proteins (PBPs)
  • Stable in presence of beta-lactamases

Dosage & Administration

  • Dosage based on suspected or confirmed pathogen susceptibility
  • Recommended regimens for adult patients with normal renal function
  • Dose reduction for patients with reduced renal function
  • Reconstitution with appropriate diluent before intravenous infusion
  • Recommended dosage in pediatric patients for non-CNS infections

Interaction

  • Concurrent administration with probenecid may increase cilastatin half-life
  • Increased risk of generalized seizures with concurrent use of ganciclovir

Contraindications

  • Hypersensitivity to any component of this combination

  • Nausea, diarrhea, vomiting
  • Rash, pruritus, urticaria
  • Pain at injection site, erythema, vein induration
  • Fever
  • Hypotension
  • Seizures, dizziness, somnolence

Pregnancy & Lactation

  • Pregnancy Category C
  • No adequate and well-controlled studies in pregnant women
  • Use during pregnancy only if potential benefit justifies potential risk
  • Caution during nursing due to unknown excretion in human milk

Precautions & Warnings

  • Serious and occasionally fatal hypersensitivity reactions reported
  • Discontinue drug in case of allergic reaction
  • Seizures and other CNS adverse reactions reported
  • Evaluate neurologically if CNS adverse reactions occur

Reconstitution

  • Do not use diluents containing benzyl alcohol for neonates
  • Reconstitution with 0.9% Sodium Chloride Injection
  • Range of color after reconstitution does not affect potency
  • Storage of reconstituted solutions for up to 4 hours at room temperature
  • Inspection for particulate matter and discoloration before administration
