Indications
- Treatment of primary biliary cholangitis (PBC) without cirrhosis
- Treatment of PBC with compensated cirrhosis without evidence of portal hypertension
- Can be used in combination with ursodeoxycholic acid (UDCA)
- Can be used as monotherapy for patients unable to tolerate UDCA
Pharmacology
- Obeticholic acid is an agonist for Farnesoid X Receptor (FXR)
- Key regulator of bile acid, inflammatory, fibrotic and metabolic pathways
- Decreases intracellular hepatocyte concentrations of bile acids
- Increases transport of bile acids out of the hepatocytes
Dosage & Administration
- Start with a dosage of 5 mg once daily for the first 3 months
- Increase to a maximum dosage of 10 mg once daily if needed
- Monitoring recommended for safety and treatment response
- Management strategies for intolerable pruritus
Interaction
- Interacts with bile acid binding resins
- Interacts with warfarin
- May increase exposure to CYP1A2 substrates with narrow therapeutic index
- Inhibitors of BSEP should be avoided
Contraindications
- Decompensated cirrhosis
- Compensated cirrhosis with evidence of portal hypertension
- Complete biliary obstruction
