Indications

  • Monotherapy for partial-onset seizures in adults and adolescents from 16 years of age with newly diagnosed epilepsy
  • Adjunctive therapy for partial-onset seizures in adults, adolescents, children, and infants from 1 month of age with epilepsy
  • Treatment of myoclonic seizures in adults and adolescents from 12 years of age with Juvenile Myoclonic Epilepsy
  • Treatment of primary generalized tonic-clonic seizures in adults and adolescents from 12 years of age with Idiopathic Generalised Epilepsy

Mechanism

The precise mechanism(s) by which Levetam exerts its antiepileptic effect is unknown. It showed minimal activity in certain tests and protection against secondarily generalized activity from focal seizures induced by certain convulsants.

Dosage

  • Recommended starting dose for adults and adolescents from 16 years of age is 250 mg twice daily, increased to 500 mg twice daily after two weeks
  • Maximum dose is 1500 mg twice daily for adults

Administration

The film-coated tablets must be taken orally, swallowed with sufficient quantity of liquid, and may be taken with or without food. The daily dose is administered in two equally divided doses.

Interaction

  • No influence on serum concentrations of existing antiepileptic medicinal products
  • Concomitant administration with methotrexate has been reported to decrease methotrexate clearance
  • Decreased Levetam efficacy reported when concomitantly administered with osmotic laxative macrogol

Contraindications

Hypersensitivity to the active substance or other pyrrolidone derivatives or to any of the excipients

Side Effects

  • Most common adverse reactions include somnolence, asthenia, infection, dizziness in adults
  • Fatigue, aggression, nasal congestion, decreased appetite, irritability in pediatric patients

Pregnancy & Lactation

Not recommended during pregnancy and breastfeeding

Precautions & Warnings

  • Dose adjustment may be required in patients with renal impairment
  • Rare cases of decreased blood cell counts reported
  • Patients should be monitored for signs of depression and suicidal ideation and behaviors

Use in Special Populations

  • Elderly: Dose adjustment recommended in elderly patients with compromised renal function
  • Renal impairment: Daily dose must be individualized according to renal function

Overdose Effects

  • Somnolence, agitation, aggression, depressed level of consciousness, respiratory depression, and coma were observed with Levetam overdoses
  • No specific antidote for Levetam
