Indications

  • Partial-onset seizures with or without secondary generalization
  • Myoclonic seizures
  • Generalized tonic-clonic seizures

Description

  • Mechanism of antiepileptic effect is unknown
  • Inhibition of burst firing without affecting normal neuronal excitability

Dosage

  • Initial starting dose of 250mg twice daily for adults and adolescents from 16 years of age
  • Maximum dose of 1500mg twice daily for adults
  • Tablet formulation not adapted for use in infants and children under the age of 6 years

Administration

  • Oral administration with sufficient liquid
  • Administered in two equally divided doses

Interaction

  • No influence on serum concentrations of existing antiepileptic drugs
  • Potential interaction with probenecid and methotrexate
  • Decreased efficacy with concomitant use of osmotic laxative macrogol

Contraindications

  • Hypersensitivity to the active substance or other pyrrolidone derivatives

Side Effects

  • Common adverse reactions include somnolence, asthenia, infection, dizziness in adults
  • Fatigue, aggression, nasal congestion, decreased appetite, irritability in pediatric patients

Pregnancy & Lactation

  • Not recommended during pregnancy and breastfeeding
  • Excreted in human breast milk

Precautions & Warnings

  • Dose adjustment in renal impairment
  • Rare cases of decreased blood cell counts described
  • Associated with acute kidney injury
  • Potential risk of suicidal thoughts and behavior
  • Not adapted for use in infants and children under 6 years

Use in Special Populations

  • Dose adjustment in elderly and renal impairment
  • No dose adjustment needed in mild to moderate hepatic impairment

Overdose Effects

  • Symptoms include somnolence, agitation, depressed level of consciousness
  • No specific antidote
  • Symptomatic treatment recommended including hemodialysis
