Indications
- Treatment of partial-onset seizures with or without secondary generalization
- Monotherapy in adults and adolescents from 16 years of age with newly diagnosed epilepsy
- Adjunctive therapy in adults, adolescents, children, and infants from 1 month of age with epilepsy
- Treatment of myoclonic seizures in adults and adolescents from 12 years of age with Juvenile Myoclonic Epilepsy
- Treatment of primary generalized tonic-clonic seizures in adults and adolescents from 12 years of age with Idiopathic Generalised Epilepsy
Description
- Precise mechanism(s) of action is unknown
- Minimal activity in submaximal stimulation and threshold tests
- Protection observed against secondarily generalized activity from focal seizures induced by pilocarpine and kainic acid
- Inhibition of burst firing without affecting normal neuronal excitability
Dosage
- Recommended starting dose of 250 mg twice daily for adults and adolescents from 16 years of age
- Initial therapeutic dose of 500 mg twice daily for adults (≥18 years) and adolescents (12 to 17 years) weighing 50 kg or more
- Maximum dose of 1500 mg twice daily
- Not adapted for use in infants and children under the age of 6 years
- Oral solution is preferred for pediatric population
Administration
- Oral administration with a sufficient quantity of liquid
- Administered in two equally divided doses
- Film-coated tablets
Interaction
- No influence on serum concentrations of existing antiepileptic medicinal products
- Concomitant administration with probenecid and methotrexate
- Decreased efficacy with concurrent use of osmotic laxative macrogol
- Minimal alteration by food, slightly reduced absorption rate
Contraindications
- Hypersensitivity to the active substance or other pyrrolidone derivatives or to any of the excipients
Side Effects
- Most common adverse reactions include somnolence, asthenia, infection, dizziness
- Pediatric patients may experience fatigue, aggression, nasal congestion, decreased appetite, irritability
Pregnancy & Lactation
- Not recommended during pregnancy and in women of childbearing potential not using contraception
- Excreted in human breast milk, breastfeeding not recommended unless clinically necessary
- No impact on fertility detected in animal studies
Precautions & Warnings
- Dose adjustment may be required for patients with renal impairment
- Rare cases of decreased blood cell counts reported
- Monitoring for signs of depression and suicidal ideation and behaviors recommended
- Tablet formulation not adapted for use in infants and children under the age of 6 years
Use in Special Populations
- Adjustment of the dose recommended in elderly patients with compromised renal function
- Daily dose must be individualized according to renal function
Overdose Effects
- Observation of somnolence, agitation, aggression, depressed level of consciousness, respiratory depression, and coma
- No specific antidote available, treatment is symptomatic
