Indications
- Monotherapy for newly diagnosed epilepsy in adults and children aged 6 years and above
- Adjunctive therapy for inadequately controlled epilepsy and seizures associated with Lennox Gastaut Syndrome
Pharmacology
- Blocks neuronal excitability to prevent seizures and migraines
- Stimulates GABA-A receptor activity and reduces glutamate activity
Dosage & Administration
- Initial target dose for topiramate monotherapy in adults with newly diagnosed epilepsy is 100 mg/day
- Maximum recommended daily dose for adults with epilepsy is 400 mg
- Recommended total daily dose of topiramate for migraine prophylaxis in adults is 100 mg/day
Interaction
- No clinically significant effect of topiramate on steady-state plasma concentrations of other antiepileptic drugs
- Topiramate should be used with caution if used in combination with alcohol and other CNS depressants

Pregnancy & Lactation

- Patients should be warned of potential visual disturbances and advised not to drive if affected
Overdose Effects
- Topiramate overdose can result in severe metabolic acidosis
- Treatment should be appropriately supportive and the patient should be well-hydrated
