Description
Anticoagulant used for prevention of atherothrombotic events, treatment of stroke, DVT, PE, and for prophylaxis of DVT after knee or hip replacement surgery.
Dosage Range
2.5 mg, 10-20 mg
Dosage Instructions
- 2.5 mg twice daily with 75-100 mg Aspirin or with Aspirin plus Clopidogrel or Ticlopidine
- 10-20 mg orally once daily with the evening meal
- 15 mg orally twice daily with food for the first 21 days for the initial treatment of acute DVT or PE, then 20 mg orally once daily with food
- 20 mg once daily with food for the prophylaxis of DVT, which may lead to PE in patients undergoing knee or hip replacement surgery
Mode of Action
Highly selective direct factor Xa inhibitor interrupting blood coagulation cascade
Interaction
Avoid concomitant use with P-gp and CYP3A4 inhibitors or with strong CYP3A4 inducers. Increased risk of bleeding with anti-platelet agents, heparin, fibrinolytic therapy, NSAIDs.
Contraindications
- Known hypersensitivity to Rivaroxaban or any of the excipients
- Active pathological bleeding
Side Effects
- Increased chance of bleeding
- Spinal or epidural hematoma
- Increased risk of stroke after discontinuation in nonvalvular atrial fibrillation
Pregnancy Category
Category C; Not recommended during pregnancy or breastfeeding due to lack of established safety and efficacy
Precautions
- Early discontinuation increases risk of thrombotic events
- Increased risk of fatal bleeding in presence of certain risk factors
- Monitor for neurological impairment in neuraxial anesthesia
- Not recommended in certain pulmonary embolism cases
Overdose Effects
May lead to hemorrhage; Limited absorption at single doses >50 mg; No specific antidote available; Consider activated charcoal for overdose
