Description

  • Nexdex is a proton pump inhibitor (PPI) used for the healing and maintenance of erosive esophagitis, treatment of symptomatic non-erosive gastroesophageal reflux disease (GERD), and relief of heartburn.

Indications

  • Healing of erosive esophagitis for up to 8 weeks
  • Maintenance of healed erosive esophagitis for up to 6 months
  • Treatment of heartburn associated with symptomatic non-erosive GERD for 4 weeks

Pharmacology

  • Dexlansoprazole suppresses gastric acid secretion by specific inhibition of the (H+/K+)-ATPase in the gastric parietal cell
  • Supplied as a Dual Delayed Release (DDR) formulation in a capsule for oral administration

Dosage

  • 30 mg Once daily for maintenance of healed erosive esophagitis and relief of heartburn
  • 30 mg Once daily for symptomatic non-erosive GERD for 4 weeks
  • 60 mg Once daily for healing of erosive esophagitis for up to 8 weeks

Administration

  • Can be taken without regard to food
  • Alternatively, capsules can be opened and granules sprinkled on one tablespoon and swallowed immediately

Interaction

  • May interact with Atazanavir, Warfarin, Tacrolimus, Clopidogrel, and Methotrexate

Contraindications

  • Contraindicated in patients with known hypersensitivity to any component of the formulation

Side Effects

  • Common side effects include diarrhea, abdominal pain, nausea, vomiting, and flatulence

Pregnancy & Lactation

  • Pregnancy Category B with no adequate and well-controlled studies in pregnant women
  • No adequate and well-controlled studies in lactating mothers

Precautions & Warnings

  • Precautions for gastric malignancy, Clostridium difficile associated diarrhea, bone fracture, hypomagnesemia, and concomitant use with Methotrexate

Use in Special Populations

  • Safety and effectiveness in patients below 12 years of age not established
  • No dose adjustment necessary for elderly patients, renal impairment, or mild hepatic impairment

Overdose Effects

  • No reports of significant overdose
  • High doses did not result in severe adverse events
