Indications

  • Hypertension
  • Treatment of essential hypertension
  • Chronic heart failure (CHF)
  • Treatment of stable mild and moderate chronic heart failure in elderly patients.

Pharmacology

  • β adrenergic receptor blocking agent
  • Inhibition of β1 and β1 adrenergic receptors
  • Lacks intrinsic sympathomimetic and membrane stabilizing activity
  • Decreased heart rate
  • Decreased myocardia contractility
  • Diminution of tonic sympathetic outflow to the periphery
  • Suppression of renin activity
  • Vasodilation and decreased peripheral vascular resistance
  • Metabolized by glucuronidation and hydroxylation by CYP2D6
  • 98% plasma protein binding to albumin
  • Absorption similar to oral solution
  • Metabolized via glucuronidation, N-dealkylation, and oxidation via cytochrome P450 2D6
  • 38-67% excretion in urine and feces
  • No significant interactions with digoxin or warfarin
  • Starting dose reduction in moderate hepatic impairment

Dosage & Administration

  • Recommended starting dose of 5 mg once daily
  • Can be increased at 2-week intervals up to 40 mg
  • No benefit from more frequent dosing regimen

Interaction

  • Use with caution when co-administered with myocardial depressants or AV conduction inhibitors
  • Not to be combined with other β-blockers
  • Closely monitor patients receiving catecholamine-depleting drugs
  • Exercise caution when co-administered with CYP2D6 inhibitors (quinidine, propafenone, fluoxetine, paroxetine, etc.)

Contraindications

  • Severe bradycardia
  • Heart block greater than first degree
  • Cardiogenic shock
  • Decompensated cardiac failure
  • Sick sinus syndrome (unless a permanent pacemaker is in place)
  • Severe hepatic impairment (Child-Pugh >B)
  • Hypersensitivity to any component of the product

Side Effects

  • Headache
  • Nausea
  • Bradycardia

Precautions & Warnings

  • Abrupt cessation of therapy precautions
  • Careful monitoring in patients with coronary artery disease
  • Caution in patients with compensated congestive heart failure
  • Not studied in patients with angina pectoris or recent MI
  • Patients with bronchospastic diseases should not receive β-blockers
  • Close monitoring perioperatively and caution with anesthesia and major surgery
  • Caution in patients with diabetes, hypoglycemia, thyrotoxicosis, peripheral vascular disease, and non-dihydropyridine calcium channel blockers
  • Use with caution in patients with severe renal impairment and moderate hepatic impairment
  • Increased reactivity to anaphylactic reactions

Use in Special Populations

  • Use with caution in patients with severe renal impairment
  • Use with caution in patients with moderate hepatic impairment
  • Contradicted in patients with severe hepatic impairment
  • Increased reactivity to anaphylactic reactions in patients with a history of severe anaphylactic reactions to allergens
