Indications
- Hypertension
- Treatment of essential hypertension
- Chronic heart failure (CHF)
- Treatment of stable mild and moderate chronic heart failure in elderly patients.
Pharmacology
- Beta adrenergic receptor blocking agent
- Inhibition of beta1 and beta1 adrenergic receptors
- Lack of intrinsic sympathomimetic and membrane stabilizing activity
- Decreased heart rate
- Decreased myocardial contractility
- Diminution of tonic sympathetic outflow to the periphery from cerebral vasomotor centers
- Suppression of renin activity
- Vasodilation and decreased peripheral vascular resistance
- Metabolized by glucuronidation and hydroxylation by CYP2D6
- Plasma protein binding of approximately 98% to albumin
- Absorption similar to an oral solution
- Mean peak plasma concentrations approximately 1.5 to 4 hours post-dosing
- Metabolized predominantly via direct glucuronidation of parent
- 38% to 67% recovery in urine and feces after administration
- No significant changes in pharmacokinetics when administered with digoxin or warfarin
Dosage & Administration
- Recommended starting dose of 5 mg once daily
- Dose can be increased at 2-week intervals up to 40 mg
- Administration with or without food
- More frequent dosing regimen unlikely to be beneficial
Interaction
- Use with care when myocardial depressants or AV conduction inhibitors are used concurrently
- Not to be combined with other beta-blockers
- Cautious use with catecholamine-depleting drugs
- Caution when co-administered with CYP2D6 inhibitors
Contraindications
- Severe bradycardia
- Heart block greater than first degree
- Cardiogenic shock
- Decompensated cardiac failure
- Sick sinus syndrome (unless a permanent pacemaker is in place)
- Severe hepatic impairment
- Hypersensitivity to any component of the product
Side Effects
- Headache
- Nausea
- Bradycardia
Precautions & Warnings
- Patients advised against abrupt discontinuation of therapy
- Severe exacerbation of angina and occurrence of myocardial infarction and ventricular arrhythmias reported
- Administration cautiously in patients with compensated congestive heart failure
- Not studied in patients with angina pectoris or recent MI
- Patients with bronchospastic diseases should not receive beta-blockers
- Close monitoring during anesthesia and major surgery
- Possible masking of hypoglycemia manifestations
- Possible masking of clinical signs of hyperthyroidism
- Possible precipitation or aggravation of symptoms of arterial insufficiency in patients with peripheral vascular disease
- Caution when used concomitantly with non-dihydropyridine calcium channel blockers
Use in Special Populations
- Caution in patients with severe renal impairment
- Caution in patients with moderate hepatic impairment
- Contraindicated in patients with severe hepatic impairment
- Increased reactivity to repeated challenges in patients with a history of severe anaphylactic reactions to allergens while taking beta-blockers
