Indications

  • Adjunct to diet and exercise

Pharmacology

  • Inhibitor of DPP-4 enzyme
  • Increases concentration of active incretin hormones
  • Stimulates insulin release from pancreatic beta cells
  • Decreases glucagon secretion from pancreatic alpha cells
  • Biguanide type oral antihyperglycemic drug
  • Lowers basal and postprandial plasma glucose
  • Improves insulin sensitivity
  • Decreases hepatic glucose production

Dosage & Administration

  • Maximum recommended dose of 2.5 mg Linagliptin and 1000 mg Metformin Hydrochloride twice daily with meals
  • Gradual dose escalation recommended
  • Individualized dosage based on effectiveness and tolerability

Interaction

  • Potential interaction with cationic drugs
  • Concomitant use of carbonic anhydrase inhibitors may induce metabolic acidosis
  • Rifampin decreases Linagliptin exposure
  • Drugs affecting glycemic control may lead to loss of control

Contraindications

  • Contraindicated in patients with renal impairment
  • Contraindicated in acute or chronic metabolic acidosis
  • Contraindicated in hypersensitivity to Linagliptin or Metformin Hydrochloride

Side Effects

  • Most common side effects: nasopharyngitis and diarrhea
  • Hypoglycemia more common in patients treated with Linera M and sulfonylureas

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Caution advised during lactation

Precautions & Warnings

  • Immediate discontinuation in case of lactic acidosis
  • Postmarketing reports of acute pancreatitis
  • Hematologic parameters monitoring annually due to potential lowering of Vitamin B12 levels

Overdose Effects

  • Usual supportive measures in event of overdose
  • Hypoglycemia reported in approximately 10% of cases
  • Lactic acidosis reported in approximately 32% of cases
