Description

  • Orally active cephalosporin antibiotic
  • Bactericidal against a wide variety of Gram-positive and Gram-negative organisms
  • Marked in-vitro bactericidal activity
  • Indicated for acute infections caused by susceptible microorganisms
  • Highly stable in the presence of beta-lactamase enzymes
  • Bactericidal against a broad spectrum of gram positive and gram negative bacteria
  • Interferes in the synthesis of bacterial cell wall
  • Usual course of treatment is 7 days
  • Not significantly modified by the presence of food
  • Increases in prothrombin times have been noted in a few patients
  • Generally well tolerated in clinical trials
  • Caution should be exercised when used during pregnancy and lactation
  • Caution should be taken in patients receiving anticoagulant therapy
  • Adverse reactions in small numbers of healthy adult volunteers receiving single doses up to 2 g did not differ from the profile seen in patients treated at the recommended doses

Indications

  • Upper Respiratory Tract Infections (URTI)
  • Lower Respiratory Tract Infections
  • Urinary Tract Infections
  • Clinical efficacy against commonly occurring pathogens such as Streptococcus pneumonia, Escherichia coli, Proteus mirabilis, Haemophilus influenzae

Pharmacology

  • Bactericidal against a broad spectrum of gram positive and gram negative bacteria
  • Interferes in the synthesis of bacterial cell wall
  • Highly stable in the presence of Beta-lactamase enzyme
  • Absorption is about 40% to 50% whether administered with or without food

Dosage & Administration

  • Usual course of treatment is 7 days
  • Recommended adult dosage is 200-400 mg daily for adults and children over 10 years
  • Recommended dosage for children is 8 mg/kg/day administered as a single dose or in two divided doses
  • Recommended children dose in typhoid is 5 mg/kg body weight twice daily for 10-14 days
  • Usual dose and schedule may be given in patients with creatinine clearances of 20 ml/min or greater
  • Dose of 200 mg once daily should not be exceeded in patients with creatinine clearance less than 20 ml/min
  • Dose and regimen for patients who are maintained on chronic ambulatory peritoneal dialysis or hemodialysis should follow the same recommendation as that for patients with creatinine clearances of less than 20 ml/min

Interaction

  • Increases in prothrombin times have been noted in a few patients
  • Care should be taken in patients receiving anticoagulant therapy

Contraindications

  • Known hypersensitivity to cephalosporin antibiotics

Side Effects

  • Gastrointestinal disturbances: diarrhea and stool changes, nausea, abdominal pain, dyspepsia, vomiting
  • Central nervous system: headache, dizziness
  • Hypersensitivity reactions: rash, pruritis, urticaria
  • Hematological and clinical chemistry: thrombocytopenia, leukopenia, eosinophilia
  • Mild transient change in liver and renal function tests
  • Miscellaneous: genital pruritis, vaginitis

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Drug should be used during pregnancy only if clearly needed
  • Caution should be exercised when administered to a nursing woman

Precautions & Warnings

  • Given with caution to patients allergic to other drugs
  • Caution in penicillin-sensitive patients
  • Caution in patients with markedly impaired renal function
  • Treatment alters normal flora of the colon and may permit overgrowth of clostridia
  • Not recommended for use in pregnancy or in nursing mothers unless considered essential by the physician

Overdose Effects

  • Gastric Lavage may be indicated
  • No specific antidote exists
  • Not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis
  • Adverse reactions at single doses up to 2 g did not differ from the profile seen in patients treated at the recommended doses
