Pharmacology
- Inhibitor of Calcium Channel Blocker
- Blocks transmembrane influx of Calcium ions into muscle cells
- Selective dilator of arterial vessels
- Dilates main coronary and systemic arteries
- Reduces blood pressure
- Causes sympathetic reflex response and tachycardia
- Direct negative inotropic effects on cardiac muscles
Dosage & Administration
- Nifedipine 10 mg initial dose for angina
- Nifedipine 10 mg initial dose for Raynaud's Phenomenon
- Nifedipine 20 mg starting dose for new patients
- Recommended dose for hypertension and angina prophylaxis is 20 mg twice daily
- Dosing range from 10 mg to 40 mg twice daily
- Patients with liver dysfunction should start with 10 mg twice daily
- No dosage adjustment required for patients with renal impairment
Interactions
- Enhanced hypotensive effect with ACE inhibitors
- Reduced plasma concentration of quinidine with anti-arrhythmics
- Possible increase in metabolism with rifampicin
- Increased plasma concentration of phenytoin with anti-epileptics
- Enhanced hypotensive effect with antipsychotics
- Possibility of severe hypotension and heart failure with β-blockers
- Possible increase in plasma concentration with cyclosporin
- Enhanced effect of muscle relaxants
- Increased metabolism with ulcer healing drugs
Contraindications
- Cardiogenic shock
- Advanced aortic stenosis
- Nursing mothers
- GI obstruction
- Inflammatory bowel disease
- Hypotension

- Increased frequency of micturation
- Eye pain
- Gum hyperplasia
- Depression
- Tremor
- Photosensitivity
- Jaundice (in few cases)
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus
Precautions & Warnings
- Tablets should be swallowed whole
- Caution required in patients with poor cardiac reserve
- Withdrawn if ischaemic pain occurs or worsens
- Use with caution in diabetic patients
- Caution required in treating patients with renal disease
