Pharmacology
- Inhibitor of Calcium Channel Blocker
- Dilator of arterial vessels
- Selective effects on dilating main coronary and systemic arteries
- Reduction of blood pressure
- Sympathetic reflex response causing tachycardia
- Increased cardiac output
- Direct negative inotropic effects on cardiac muscles
Dosage & Administration
- Dosage of Nifedipine 10 mg for angina
- Dosage of Nifedipine 10 mg for Raynaud's Phenomenon
- Dosage of Nifedipine 20 mg for hypertension and angina prophylaxis
- Dosage adjustment for patients with liver dysfunction
- No dosage adjustment for patients with renal impairment
Interaction
- Enhanced hypotensive effect with ACE inhibitors
- Reduced plasma concentration of quinidine with anti-arrythmics
- Possible increase in metabolism with Rifampicin
- Increased plasma concentration of phenytoin with anti-epileptics
- Enhanced hypotensive effect with antipsychotics
- Occasional severe hypotension and heart failure with β-blockers
- Possible increase in plasma concentration of Nifin with Cyclosporin
- Increased effect of muscle relaxants e.g. tubocurarine
- Increase in metabolism of Nifin with ulcer healing drugs
Contraindications
- Cardiogenic shock
- Advanced aortic stenosis
- Nursing mothers
- GI obstruction
- Inflammatory bowel disease
- Hypotension
Side Effects
- Headache
- Flushing
- Lethargy
- Gravitational edema rash
- Nausea
- Increased frequency of micturation
- Eye pain
- Gum hyperplasia
- Depression
- Tremor
- Photosensitivity
- Jaundice
- Induction of attacks of ischaemic pain in some patients with angina pectoris
Pregnancy & Lactation
- No adequate and well controlled studies in pregnant women
- Use during pregnancy only if potential benefit justifies potential risk to the fetus
Precautions & Warnings
- Tablets should be swallowed whole
- Not to be bitten, chewed or broken up
- Caution in patients with poor cardiac reserve
- Withdrawal if ischaemic pain occurs or worsens
- Control adjustment required for diabetic patients
- Exercise caution in treating patients with renal disease
