Indications

  • Osteoarthritis
  • Rheumatoid arthritis
  • Ankylosing spondylitis
  • Decreasing risk of developing gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Inhibition of prostaglandin synthesis
  • Proton pump inhibitor suppressing gastric acid secretion

Dosage

  • Lowest effective dose
  • Twice daily dosage for different conditions
  • Dosage for juvenile idiopathic arthritis based on weight

Administration

  • Tablet to be taken at least 30 minutes before meals

Interaction

  • Concomitant use of NSAIDs reducing antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
  • Increased risk of bleeding complication with warfarin
  • Inhibition of gastric acid secretion interfering with drug absorption
  • Effects of administration with high-fat food on absorption

Contraindications

  • Known hypersensitivity to components
  • History of allergic-type reactions after taking aspirin or other NSAIDs
  • Use during peri-operative period in the setting of coronary artery bypass graft surgery

Side Effects

  • Decrease in occurrence of gastric ulcers and NSAID associated adverse events
  • Increased risk of arterial thrombotic events at high doses
  • Gastrointestinal adverse events
  • Oedema, hypertension, cardiac failure
  • Nausea, vomiting, diarrhoea, flatulence, constipation, dyspepsia, abdominal pain, gastrointestinal bleeding, ulcerative stomatitis, exacerbation of colitis and Crohn’s disease

Pregnancy & Lactation

  • Contradiction during third trimester of pregnancy
  • Unknown excretion of esomeprazole in human milk
  • Impairment of female fertility
  • Not recommended for women attempting to conceive

Precautions & Warnings

  • Avoiding combination with other NSAIDs
  • Usage with low dose acetylsalicylic acid
  • Risk-factors for NSAID related gastro-intestinal complications
  • Increased frequency of adverse reactions in older people
  • Consideration for patients with GI toxicity
  • Monitoring and advice required for patients with history of hypertension and/or mild to moderate congestive heart failure

Use in Special Populations

  • Increased unbound plasma fraction of naproxen in elderly
  • Not recommended usage in patients with moderate to severe or severe renal impairment
  • Close monitoring for patients with mild to moderate hepatic impairment

Overdose Effects
