Indications
- Treatment of anemia in Chronic Kidney Disease (CKD) patients on dialysis and not on dialysis
- Treatment of anemia due to Zidovudine in HIV-infected patients
- Treatment of anemia due to concomitant myelosuppressive chemotherapy
- Reduction of allogeneic RBC transfusions in patients undergoing elective, noncardiac, nonvascular surgery
- Prevention of anemia of prematurity in infants with specified birth weight and gestational age
Description
- Recombinant human erythropoietin (EPO)
- Expressed in Chinese hamster ovary cells
- Has a 165 amino acid sequence identical to that of human urinary EPO
- Apparent molecular weight of erythropoietin is about 30,400 daltons
Dosage
- Evaluation of Iron Stores and Nutritional Factors before and during treatment
- Monitoring hemoglobin levels at least weekly until stable, then at least monthly
- Adjusting therapy based on hemoglobin rate of rise, ESA responsiveness, and hemoglobin variability
- Initiating Erythropoietin alfa treatment based on hemoglobin levels
- Dose adjustments for patients with CKD on dialysis and not on dialysis
- Starting dose for Zidovudine-treated HIV-infected Patients
- Dosing adjustments and discontinuation criteria for patients on Cancer Chemotherapy
- Dosing regimen for prevention of anemia of prematurity

Contraindications
- Uncontrolled hypertension
- Serious allergic reactions to Erythropoietin alfa
- Patients developing Pure Red Cell Aplasia (PRCA) following treatment with any erythropoietin
Side Effects
- Adverse reactions observed in Epomax treated patients in clinical studies
- Specific adverse reactions for patients with CKD, Zidovudine-treated HIV-infected Patients, Cancer Patients on Chemotherapy, Surgery Patients, and Premature infants
Pregnancy & Lactation
- Absence of adequate and well-controlled studies in pregnant women
- Reproduction toxicology observed in animal studies
- Caution advised in pregnancy and lactation
- Unknown excretion of exogenous Erythropoietin alfa in human milk
