Indications
- Treatment of HR-positive, HER2-negative advanced breast cancer
- Combination with aromatase inhibitor in postmenopausal women
- Combination with Fulvestrant in women with disease progression following endocrine therapy
Pharmacology
- Inhibitor of Cyclin-dependent kinases (CDK) 4 and 6
- Reduces cellular proliferation of ER-positive breast cancer cell lines
- Block progression of the cell from G1 into S phase of the cell cycle
Dosage & Administration
- Recommended dose of 125mg capsule taken orally once daily for 21 consecutive days
- Followed by 7 days off treatment to comprise a complete cycle of 28 days
- Should be taken with food
Interaction
- Primarily metabolized by CYP3A and Sulfotransferase (SULT) enzyme SULT2A1
- Time-dependent inhibitor of CYP3A
Side Effects
- Common side effects include decreased WBC, neutrophils, platelets, infections, AST and ALT increased
- Also includes leukopenia, fatigue, nausea, hair loss, inflammation of the mouth and lips, diarrhea, anemia, rash, weakness/lethargy, vomiting, thrombocytopenia, dry skin, and fever
Pregnancy & Lactation
- Can cause fetal harm when administered to a pregnant woman
- Risk of major birth defects and miscarriage
Precautions & Warnings
- Neutropenia was the most frequently reported adverse reaction
- Monitor complete blood counts prior to Palbo therapy and at the beginning of each cycle
Overdose Effects
- No known antidote for Palbo
- Treatment of overdose should consist of general supportive measures
