Indications
- Management of hyperthyroidism
- Treatment of thyrotoxicosis
- Preparation of patients for thyroidectomy
- Combination therapy with radio-active ablative therapy
Pharmacology
- Depresses the formation of thyroid hormone
- Reduces uptake and concentration of inorganic iodine by the thyroid
- Reduces formation of di-iodotyrosine and thyroxine
- Rapid absorption from the gastro-intestinal tract
- Complete metabolism to methimazole
- Crosses placental barrier and high concentration in lactating patient's milk
Dosage & Administration
- Initial dose: 20-60 mg in 2-3 divided doses until euthyroid
- Maintenance dose: gradually reduced to maintain euthyroid state (5-15 mg/day)
- Initial dose for neonates & children below 12 years: 250 mcg/kg/day in divided doses
- Duration of treatment: 18 to 24 months
Interaction
- Decreased response to Carbizol with iodine or iodine excess
- Possible need for increased dosage or longer duration with antithyroid agents
- Effect on response to oral anticoagulants
- Enhanced anticoagulant effects with thioamide-induced hypoprothrombinemia
- Possible increase in serum concentrations of digoxin and digitoxin
Contraindications
- Hypersensitivity to carbimazole or other thiourea antithyroid agents

Pregnancy & Lactation
- Use in pregnancy with smallest effective dose
- Potential adverse effects on the fetus
- Potential to cause foetal or neonatal hypothyroidism and goitre
Precautions & Warnings
- Caution with tracheal obstruction
- High dosages may produce thyroid enlargement
Overdose Effects
- Hypothyroidism
- Goitre
- Potential blood dyscrasias
- Immediate withdrawal if blood dyscrasias occur
- Symptomatic and supportive treatment
