Interactions

  • Plasma level increase with CYP3A4 inhibitors
  • Plasma level decrease with CYP3A4 inducers
  • Increased risk of bleeding with aspirin
  • Potentiation of other medication effects
  • Potentiation of hypotensive effect with diuretics, antihypertensives, vasodilators
  • Potentiation of neurotoxic effect of lithium

Contraindications

  • Severe left ventricular dysfunction
  • Hypotension or cardiogenic shock
  • Sick sinus syndrome
  • Second or third-degree atrioventricular (AV) block
  • Patients with atrial flutter or atrial fibrillation and an accessory by pass tract
  • Known hypersensitivity to verapamil hydrochloride

  • Hypotension <90 mm Hg systolic pressure
  • Atrial fibrillation/flutter
  • Simultaneous pre-excitation syndrome eg. WPW syndrome
  • Care when driving or operating machinery
  • Prolongs effects of alcohol

Overdose Effects

  • Supportive treatment
  • Possible use of beta-adrenergic stimulation
  • Possible use of parenteral administration of calcium solution
  • Hemodialysis ineffective for removal
