Medicine Details

Indications

  • Adjunct to diet and exercise in type 2 diabetes mellitus patients
  • Monotherapy with glimepiride or metformin not resulting in adequate glycemic control
  • Replacement of combination therapy of glimepiride and metformin

Pharmacology

  • Decreases blood glucose concentration
  • Stimulates release of insulin from functioning pancreatic beta cells
  • Improves sensitivity of beta cells to glucose stimulus
  • Reduces basal hepatic glucose production
  • Increases peripheral tissue sensitivity to insulin
  • Improves insulin sensitivity by increasing peripheral glucose uptake and utilization
  • Lowers both basal and postprandial plasma glucose
  • Does not produce hypoglycemia
  • Decreases hepatic glucose production
  • Decreases intestinal absorption of glucose

Dosage

  • Governed by desired blood glucose level
  • Must be the lowest sufficient to achieve desired metabolic control
  • Regular measurement of glucose levels in blood and urine
  • Avoid correcting mistakes by taking larger dose
  • Timely dose reduction or cessation to avoid hypoglycemia
  • The highest recommended dose per day should be 8 mg of glimepiride and 2000 mg of metformin
  • Starting dose should not exceed the daily doses of glimepiride or metformin already being taken
  • When switching from combination therapy, administer on basis of current dosage

Administration

  • Must be swallowed whole and not crushed or chewed

Interaction

  • Metabolized by CYP2C9
  • Potentiation and hypoglycemia with drugs like insulin, oral antidiabetics, ACE inhibitors, anabolic steroids
  • Weakening and raised blood glucose levels with drugs like barbiturates, corticosteroids, diuretics
  • H2 receptor antagonists, beta-blockers, clonidine, and reserpine may lead to either potentiation or weakening of blood-glucose-lowering effect
  • Alcohol intake may potentiate or weaken blood-glucose-lowering action in an unpredictable fashion

Contraindications

  • Hypersensitivity to glimepiride, metformin, sulfonamides, or any excipients
  • In pregnant and breastfeeding women
  • Severe impairment of liver function and in dialysis patients
  • Acute metabolic acidosis, severe renal failure, acute conditions altering renal function, hepatic insufficiency, acute alcohol intoxication, lactation

Side Effects

  • Hypoglycemia, prolonged hypoglycemia, visual impairment, gastrointestinal symptoms, hepatitis, dysgeusia, changes in blood picture, alopecia, allergic reactions, weight gain
  • Gastrointestinal symptoms, metallic taste, decrease of vitamin B12 absorption, lactic acidosis, hemolytic anemia, reduction of thyrotropin level, hypomagnesemia

Pregnancy & Lactation

  • Glimepiride must not be taken during pregnancy, patients should change over to insulin
  • Metformin should not be used in pregnancy, diabetes should be treated with insulin to maintain blood glucose levels as close to normal as possible
  • Glimepiride must not be taken by breastfeeding women, patients must change over to insulin
  • Metformin is excreted into milk in lactating rats, a decision should be made whether to discontinue nursing or discontinue metformin

Precautions & Warnings

  • Initial weeks of treatment may increase risk of hypoglycemia, especially when illness occurs or lifestyle changes
  • Regular monitoring of thyroid-stimulating hormone (TSH) levels is recommended in patients with hypothyroidism
  • Long-term treatment associated with decrease in vitamin B12 serum levels leading to peripheral neuropathy

Use in Special Populations

  • Insufficient data to recommend pediatric use
  • Assessment of GFR before initiation of treatment
  • Renal function assessment in patients at increased risk, especially in the elderly
  • Maximum daily dose based on GFR levels

Overdose Effects

  • Acute overdosage may lead to severe life-threatening hypoglycemia
  • High overdose may lead to lactic acidosis, pancreatitis
