Medicine Details

Indications

  • Adjunct to diet and exercise in type 2 diabetes mellitus patients
  • For inadequate glycemic control with monotherapy of glimepiride or metformin
  • Replacement of combination therapy of glimepiride and metformin

Pharmacology

  • Sulfonylurea antidiabetic agent
  • Stimulates release of insulin
  • Improves sensitivity of beta cells to glucose stimulus
  • Reduces basal hepatic glucose production
  • Increases peripheral tissue sensitivity to insulin
  • Decreases intestinal absorption of glucose
  • Improves insulin sensitivity

Dosage

  • Governed by desired blood glucose level
  • Lowest dosage sufficient for desired metabolic control
  • Regular measurement of glucose levels in blood and urine
  • Highest recommended dose per day: 8 mg of glimepiride and 2000 mg of metformin

Administration

  • Swallowed whole
  • Not crushed or chewed

Interaction

  • Metabolized by cytochrome P450 2C9 (CYP2C9)
  • Potentiation of blood-glucose-lowering effect with insulin, ACE inhibitors, anabolic steroids, etc.
  • Weakening of blood-glucose-lowering effect with corticosteroids, diuretics, phenytoin, etc.
  • Alcohol intake may potentiate or weaken the blood-glucose-lowering action
  • Bile acid sequestrant (Colesevelam) binds to glimepiride

Contraindications

  • Hypersensitivity to glimepiride, metformin, sulfonylureas, sulfonamides, or excipients
  • Pregnant or breastfeeding women
  • Severe impairment of liver function
  • Severe renal failure
  • Acute metabolic acidosis
  • Tissue hypoxia

Side Effects

  • Hypoglycaemia
  • Temporary visual impairment
  • Nausea, vomiting, abdominal pain
  • Hepatitis, elevation of liver enzyme levels
  • Blood disorders (thrombocytopenia, leucopenia, anemia)
  • Alopecia
  • Allergic or pseudo-allergic reactions
  • Weight gain (associated with sulfonylureas)
  • Gastrointestinal symptoms, metallic taste
  • Decreased vitamin B12 absorption
  • Lactic acidosis, hemolytic anemia, hypomagnesemia

Pregnancy & Lactation

  • Glimepiride must not be taken during pregnancy or lactation
  • Metformin should not be used during pregnancy; insulin is recommended
  • Metformin excreted into milk in lactating rats, decision should be made regarding nursing

Precautions & Warnings

  • Initial weeks of treatment may increase risk of hypoglycemia
  • Caution in patients with G6PD-deficiency
  • Regular monitoring of thyroid-stimulating hormone levels
  • Long-term use associated with decrease in vitamin B12 levels

Use in Special Populations

  • Insufficient data for pediatric use
  • Renal impairment: Regular assessment of GFR, dose adjustments based on GFR levels

Overdose Effects

  • Acute overdosage may lead to severe hypoglycemia
  • Long-term treatment with high dose may also lead to severe hypoglycemia
  • Lactic acidosis is a medical emergency and must be treated in hospital
