Indications
- Adjunct to diet and exercise in type 2 diabetes mellitus patients
- Monotherapy with glimepiride or metformin does not result in adequate glycemic control
- Replacement of combination therapy of glimepiride and metformin
Pharmacology
- Decreases blood glucose concentration
- Stimulates the release of insulin from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Improves glucose uptake
Dosage
- Determined by desired blood glucose level
- Lowest dosage sufficient for desired metabolic control
- Regular measurement of glucose levels in blood and urine
- Maximum recommended dose of 8 mg glimepiride and 2000 mg metformin per day
Administration
- Must be swallowed whole
- Must not be crushed or chewed
Interaction
- Metabolized by cytochrome P450 2C9 (CYP2C9)
- Coadministration with inducers or inhibitors of CYP 2C9
- Potential potentiation of blood-glucose-lowering effect
- Possible hypoglycemia with certain drug combinations
Contraindications
- Hypersensitivity to glimepiride, metformin, or other sulfonylureas
- Pregnancy
- Breastfeeding
- Severe impairment of liver function
- Severe Renal failure
- Acute metabolic acidosis

- Gastrointestinal symptoms
- Liver enzyme elevation
- Blood picture changes
- Skin disorders
- Allergic or pseudo-allergic reactions
- Weight gain
- Nausea, vomiting, diarrhoea, abdominal pain with metformin
- Metallic taste
- Decrease of vitamin B12 absorption
Pregnancy & Lactation
- Glimepiride not recommended during pregnancy
- Metformin not recommended during pregnancy
- Glimepiride not recommended for breastfeeding women
- Metformin excreted into milk
Precautions & Warnings
- Increased risk of hypoglycemia in initial weeks of treatment
- Careful monitoring of hypoglycemia risk factors
- Caution in patients with G6PD-deficiency
- Regular monitoring of thyroid-stimulating hormone (TSH) levels in hypothyroidism patients
- Long-term treatment associated with decrease in vitamin B12 serum levels
Use in Special Populations
- Insufficient data to recommend pediatric use
- Assessment of GFR before initiation of treatment with metformin-containing products
- Regular renal function assessment in patients at increased risk
Overdose Effects
- Acute life-threatening hypoglycemia with glimepiride overdose
- Immediate treatment of severe reactions with glucose or glucagon
- Lactic acidosis with high overdose or concomitant risks of metformin
