Indications
- Prevention of angina pectoris
- Long-term angina treatment
Pharmacology
- Dilates arterioles
- Opens potassium channels
- Stimulates guanylate cyclase
- Causes venous vasodilatation
- Reduces preload and afterload
- Improves coronary blood flow
Dosage & Administration
- Usual therapeutic range: 10-20 mg twice daily
- Starting dose: 10 mg twice daily
- Titration upwards in accordance with patient's needs
- Range up to 40 mg twice daily if necessary
- Lower starting dose of 5 mg twice daily for patients prone to headache
- No special dosage requirements for elderly patients
- Not recommended for children
- Dose reduction may be necessary for patients with hepatic impairment
Interactions
- No observed interaction with beta-blockers, calcium antagonists, digoxin, rifampicin, and cimetidine
- Potential potentiation with other vasodilators, tricyclic antidepressants, and antihypertensive drugs
- Caution advised when combined with alcohol
Contraindications
- Hypersensitivity to Nicorandil
- Cardiogenic shock
- Acute myocardial infarction with acute left ventricular failure and low filling pressures
- Hypotension
- Concurrent use with phosphodiesterase-5 inhibitors
- Caution advised for patients with blood volume depletion, low systolic blood pressure, glaucoma, or taking other hypotensive medications
