Indications
- Treatment of chronic angina
- Should be used with beta-blockers, nitrates, calcium channel blockers, anti-platelet therapy, lipid-lowering therapy, ACE inhibitors, and angiotensin receptor blockers
- Decreases angina episodes in patients with coronary artery disease
- Effect on angina rate or exercise tolerance is smaller in women than men
Pharmacology
- Has anti-ischemic and antianginal effects
- Does not depend upon reductions in heart rate or blood pressure
- Inhibits the cardiac late sodium current (INa)
- Prolongs the QT interval
- Inhibits IkR prolonging the ventricular action potential
- Absorption: Cmax - 2-5 h, Half-life: 6-22 h, Distribution: 62% bound to human plasma proteins, Metabolism and Excretion: Metabolized mainly by CYP3A and CYP2D6, Rapid and extensive metabolism in the liver and intestine
Dosage & Administration
- Initiate at 500 mg twice daily and increase to 1000 mg twice daily if needed
- Take with or without meals
- Swallow whole, do not crush, break or chew
- Maximum recommended daily dose is 1000 mg twice daily
- Safety and effectiveness not established for pediatric use
Interaction
- Avoid use with strong CYP 3A inhibitors
- Limit maximum dose with moderate CYP 3A inhibitors
- Do not use with CYP 3A inducers
- May need to lower dose with P-gp inhibitors
- May require reduced doses when used with other drugs
Contraindications
- Pre-existing QT prolongation
- Hepatic impairment
- Taking QT prolonging drugs
- Taking potent and moderately potent CYP3A inhibitors

- Bradycardia
- Palpitations
- Tinnitus
- Vertigo
- Abdominal pain
- Dry mouth
- Vomiting
- Peripheral edema
- Dyspnea
- Hypotension
- Orthostatic hypotension
Pregnancy & Lactation
- Pregnancy Category C
- No adequate studies assessing the effect on the developing fetus
- No adequate well-controlled studies in pregnant women
- Use during pregnancy only if potential benefit justifies potential risk to the fetus
- Not known whether excreted in human milk
- Consideration for nursing mother
Precautions & Warnings
- Blocks QTc and prolongs the QTc interval in a dose-related manner
- No increased risk of proarrhythmia or sudden death in acute coronary syndrome population
- Increased plasma concentrations of digoxin when co-administered
- Caution when co-administering with P-gp inhibitors
- Exercise caution when co-administering with P-gp inhibitors such as ritonavir or cyclosporine
Overdose Effects
- Dose-related increases in dizziness, nausea, and vomiting
- Produces diplopia, paresthesia, confusion, and syncope
- Continuous ECG monitoring may be warranted
- Hemodialysis is unlikely to be effective
