Indications
- Treatment of hypertension
- Treatment of left ventricular failure
- Treatment of Raynaud's phenomenon and Raynaud's disease
- Adjunct in the symptomatic treatment of urinary obstruction caused by benign prostatic hyperplasia
Pharmacology
- Selective inhibition of postsynaptic alpha-1-adrenoreceptors in vascular smooth muscle
- Reduction in total peripheral vascular resistance
- Reduction in left ventricular filling pressure
- Reduction in cardiac impedance
- Augmentation of cardiac output
Dosage & Administration
- Adjust dosage based on patient's individual toleration
- Starting dose for hypertension: 0.5 mg once daily
- Starting dose for left ventricular failure: 0.5 mg two, three, or four times a day
- Starting dose for Raynaud's phenomenon/disease: 0.5 mg twice daily
- Starting dose for benign prostatic hyperplasia: 0.5 mg twice daily
- Dosage titration steps may be performed every two or three days
Interaction
- No adverse drug interaction reported with various classes of drugs including cardiac glycosides, hypoglycemic agents, tranquilizers, antiarrhythmic agents, and analgesics
Contraindications
- Known sensitivity to quinazolines
Side Effects
- Common reactions: dizziness, headache, drowsiness, lack of energy, weakness, palpitations, nausea
- Other reactions: vomiting, diarrhea, edema, orthostatic hypotension, dyspnea, faintness, tachycardia, nervousness, rash, alopecia, urinary frequency, impotence, blurred vision, epistaxis, dry mouth, fever
Pregnancy & Lactation
- No teratogenic effects seen in animal testing
- Excreted in small amounts in human milk
Precautions & Warnings
- Caution advised for patients with postural hypotension
- Careful adjustment of diuretic dosage in left ventricular failure patients
- Close monitoring of blood pressure during administration for Raynaud's phenomenon/disease and benign prostatic hyperplasia
- Not recommended for use in children under 12 years
Use in Special Populations
- Not recommended in the treatment of left ventricular failure due to mechanical obstruction or recent myocardial infarction
Overdose Effects
- Support of the cardiovascular system is of first importance
- Renal function should be monitored and supported
