Indications
- Adjunct to diet and exercise for type 2 diabetes mellitus
- Reducing risk of cardiovascular events in diabetes mellitus patients
- Weight management for obesity patients with comorbid conditions
Pharmacology
- GLP-1 analogue
- Stimulates insulin secretion
- Reduces blood glucose in a glucose-dependent manner
- Reduces appetite and body weight
- Delays gastric emptying
Dosage and Administration
- Tablet to be taken at least 30 minutes before first intake of the day
- Starter dose of 3 mg once daily for 30 days
- Maintenance dose of 7 mg once daily for diabetes
- Injection starting dose of 0.25 mg once weekly, dosage escalation
- Weekly administration at any time with or without meals
- Switching patients between tablet and injection
Interaction
- Delays gastric emptying
- Potential impact on oral medicinal product absorption
- Caution with oral meds requiring rapid GI absorption
Contraindications
- Hypersensitivity to active substance or excipients
Side Effects
- Hypoglycemia with insulin or sulfonylurea
- Gastrointestinal disorders
- Nausea, diarrhea, vomiting, abdominal pain, constipation
- Allergic reactions, injection site reactions, lipodystrophy, pruritus, rash
Pregnancy and Lactation
- Not to be used during pregnancy
- Discontinue at least 2 months before planned pregnancy
- Avoid during breastfeeding
Precautions and Warnings
- Not for type 1 diabetes or diabetic ketoacidosis
- Prompt discontinuation in suspected pancreatitis
- Monitoring for diabetic retinopathy
Use in Special Populations
- Safety and efficacy in children and adolescents not established
- No dose adjustment based on age
- No dose adjustment for renal impairment except end-stage
- No dose adjustment for hepatic impairment, caution needed
Overdose Effects
- Reported overdoses up to 4 mg in a single dose and week
- Most common reaction is nausea
- No specific antidote, supportive treatment for symptoms
- Dose adjustment with other diabetes medications
