Pharmacology

  • Dihydropyridine calcium-channel blocker
  • Long duration of action
  • Influences myocardial cells
  • Affects specialized conducting system of the heart
  • Affects cells of vascular smooth muscle
  • Results in vasodilation
  • Reduces peripheral resistance
  • Increases coronary blood flow
  • Increases myocardial oxygen supply
  • Increases cardiac output

Dosage & Administration

  • Usual dose of 5 mg once daily for hypertension
  • Maximum dose of 10 mg once daily for hypertension
  • Usual dose of 5 to 10 mg for angina
  • Lower dose for elderly patients with hepatic insufficiency
  • May be taken without regard to meals

Interaction

  • Potentially hazardous interactions with p-blockers in patients with impaired cardiac left ventricular function
  • No interaction with digoxin
  • No interaction with cimetidine
  • No significant alteration of warfarin effect on prothrombin time
  • Food does not alter absorption rate

Contraindications

  • Hypersensitivity to dihydropyridine derivatives
  • Pregnancy

Side Effects

  • Dizziness
  • Flushing
  • Headache
  • Hypotension
  • Peripheral edema
  • Gastrointestinal disturbances
  • Increased micturition frequency
  • Lethargy
  • Eye pain
  • Mental depression
  • Paradoxical increase in ischaemic chest pain
  • Excessive fall in blood pressure
  • Cerebral or myocardial ischaemia
  • Transient blindness
  • Rashes
  • Fever
  • Abnormalities in liver function

Pregnancy & Lactation

  • Pregnancy Category C
  • No adequate and well-controlled studies in pregnant women
  • Use during pregnancy justified by potential benefit
  • Not known whether excreted in human milk
  • Recommendation to discontinue nursing during administration

Precautions & Warnings

  • Precaution in patients with hepatic impairment
  • Precaution during pregnancy
  • Precaution during breastfeeding

Use in Special Populations

  • Starting dose of 2.5 mg once daily for children with hypertension from 6 years to 17 years of age
  • Up-titrated dose to 5 mg once daily for children with hypertension if blood pressure goal not achieved
  • Effect on blood pressure in patients less than 6 years of age is not known
  • Normal dosage regimens recommended in the elderly
  • Careful increase of dosage in the elderly
  • Normal dosage recommended for renal impairment
  • Dosage caution in mild to moderate hepatic impairment
  • Initiation at lowest dose for severe hepatic impairment

Overdose Effects

  • Excessive peripheral vasodilatation
  • Reflex tachycardia
  • Systemic hypotension
  • Shock with fatal outcome reported
  • Active cardiovascular support required
  • Frequent monitoring of cardiac and respiratory function
  • Elevation of extremities
  • Attention to circulating fluid volume and urine output
  • Vasoconstrictor may be helpful in restoring vascular tone
  • Intravenous calcium gluconate may be beneficial
  • Gastric lavage may be worthwhile in some cases
  • Charcoal shown to reduce absorption rate
  • Dialysis not likely to be of benefit
