Dosage & Administration
- Usual dose of 5 mg once daily for hypertension
- Maximum dose of 10 mg once daily for hypertension
- Usual dose of 5 to 10 mg for angina
- Lower dose for elderly patients with hepatic insufficiency
- May be taken without regard to meals
Interaction
- Potentially hazardous interactions with p-blockers in patients with impaired cardiac left ventricular function
- No interaction with digoxin
- No interaction with cimetidine
- No significant alteration of warfarin effect on prothrombin time
- Food does not alter absorption rate
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnancy
Side Effects
- Dizziness
- Flushing
- Headache
- Hypotension
- Peripheral edema
- Gastrointestinal disturbances
- Increased micturition frequency
- Lethargy
- Eye pain
- Mental depression
- Paradoxical increase in ischaemic chest pain
- Excessive fall in blood pressure
- Cerebral or myocardial ischaemia
- Transient blindness
- Rashes
- Fever
- Abnormalities in liver function
Pregnancy & Lactation
- Pregnancy Category C
- No adequate and well-controlled studies in pregnant women
- Use during pregnancy justified by potential benefit
- Not known whether excreted in human milk
- Recommendation to discontinue nursing during administration
Precautions & Warnings
- Precaution in patients with hepatic impairment
- Precaution during pregnancy
- Precaution during breastfeeding
Use in Special Populations
- Starting dose of 2.5 mg once daily for children with hypertension from 6 years to 17 years of age
- Up-titrated dose to 5 mg once daily for children with hypertension if blood pressure goal not achieved
- Effect on blood pressure in patients less than 6 years of age is not known
- Normal dosage regimens recommended in the elderly
- Careful increase of dosage in the elderly
- Normal dosage recommended for renal impairment
- Dosage caution in mild to moderate hepatic impairment
- Initiation at lowest dose for severe hepatic impairment
Overdose Effects
- Excessive peripheral vasodilatation
- Reflex tachycardia
- Systemic hypotension
- Shock with fatal outcome reported
- Active cardiovascular support required
- Frequent monitoring of cardiac and respiratory function
- Elevation of extremities
- Attention to circulating fluid volume and urine output
- Vasoconstrictor may be helpful in restoring vascular tone
- Intravenous calcium gluconate may be beneficial
- Gastric lavage may be worthwhile in some cases
- Charcoal shown to reduce absorption rate
- Dialysis not likely to be of benefit
