Indications

Pharmacology
- Inhibitor of gastric acid secretion
- Blocks hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system
- Onset of antisecretory effect within one hour
- Maximum effect occurring within two hours
- Inhibition of secretion lasts up to 72 hours
- Secretory activity returns gradually over 3 to 5 days after discontinuation

Administration
- Direction for use of IV Injection
- Direction for use of IV Infusion
- IV injection should be given as a slow intravenous injection
- IV infusion should be given as an intravenous infusion over a period of 20-30 minutes or more
- Reconstituted solution should not be mixed or co-administered with any other drug in the same infusion set
Interaction
- Decreased intragastric acidity may reduce the absorption of ketoconazole
- Metabolized in the liver through cytochrome P450
- Can delay the elimination of diazepam, phenytoin, and warfarin
- Plasma concentrations of Peplo and clarithromycin are increased during concomitant administration
- No evidence of an interaction with phenacetin, theophylline, caffeine, propranolol, metoprolol, cyclosporin, lidocaine, quinidine, estradiol, amoxycillin, or antacids
- Absorption not affected by alcohol or food
- Concomitant treatment with digoxin leads to a 10% increase in the bioavailability of digoxin
Contraindications
- Known hypersensitivity to Omeprazole
- Suspicion of gastric ulcer should exclude the possibility of malignancy before treatment
Side Effects
- Mild and reversible adverse reactions
- Skin rash, urticaria, and pruritus reported
- Photosensitivity, bullous eruption, erythema multiforme, angioedema, and alopecia reported in isolated cases
- Diarrhea and headache reported
- Gastrointestinal reactions including constipation, nausea/vomiting, flatulence, and abdominal pain
- Other reactions include dry mouth, stomatitis, candidiasis, paraesthesia, dizziness, light headedness, and feeling faint
- Reversible mental confusion, agitation, depression, and hallucinations reported predominantly in severely ill patients
- Arthritic and myalgic symptoms reported
- Isolated cases of vision disturbance, taste disturbance, peripheral edema, and increased sweating
- Isolated cases of leucopenia, thrombocytopenia, agranulocytosis, pancytopenia, anaphylactic shock, malaise, fever, bronchospasm, encephalopathy, hepatitis, interstitial nephritis, and hepatic failure
- Increases in liver enzymes observed
Pregnancy & Lactation
- No adverse effects of Omeprazole on pregnancy or on the health of the fetus/newborn child
- Omeprazole can be used during pregnancy
- No information available on the passage of Omeprazole into breast milk or its effects on the neonate
- Breast-feeding should be discontinued if the use of Omeprazole is essential
Precautions & Warnings
- Avoid concomitant use of clopidogrel and Peplo
- Observational studies suggest PPI therapy may be associated with an increased risk for osteoporosis-related fractures
- Atrophic gastritis noted occasionally in gastric corpus biopsies from long-term treated patients
- Concomitant use of PPIs with methotrexate may lead to methotrexate toxicities
