Indications

  • treatment of hypertension

Pharmacology

  • Telmisartan blocks vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • specifically blocks the binding of angiotensin II to the AT1 receptor
  • does not inhibit ACE or bind to other hormone receptors or ion channels
  • Hydrochlorothiazide affects renal tubular mechanisms of electrolyte reabsorption
  • increases excretion of sodium salt and chloride
  • reduces plasma volume
  • increases plasma renin activity
  • increases aldosterone secretion
  • reduces potassium loss
  • mechanism of antihypertensive effect is not fully understood

Dosage & Administration

  • Initiate with Telmisartan monotherapy 80 mg or Hydrochlorothiazide 25 mg once daily
  • Telmisartan and Hydrochlorothiazide 80 mg/12.5 mg once daily
  • Dose can be titrated up to 160 mg/25 mg after 2 to 4 weeks
  • may be administered with other antihypertensive drugs

Interaction

  • Caution with aliskiren, digoxin, lithium, other high blood pressure medicines, NSAIDs, corticosteroids, ACE blockers, angiotensin II receptor blockers
  • may interfere with certain laboratory tests

Contraindications

  • hypersensitivity to Telmisartan, Hydrochlorothiazide, or any other component
  • do not co-administer with aliskiren in diabetic patients
