Indications

• Treatment of HIV-1 infection
• Treatment of chronic hepatitis B in adults

Mechanism of Action

Inhibition of HIV reverse transcriptase

Pharmacokinetics

• Oral bioavailability of approximately 25%
• Max serum concentrations achieved in 1.0-10.4 hrs
• Approximately 17 hours half-life

Dosage & Administration

• Adjustable dosage to individual patient's needs
• Administered orally once daily
• Optimal duration for chronic hepatitis B treatment unknown

Interaction

• Coadministration caution with didanosine
• Monitoring needed for atazanavir and lopinavir/ritonavir

Contraindications

• Broken bone due to disease or illness
• Enlarged fatty liver
• Severe liver disease
• Acute kidney disease
• Decreased calcification or density of bone
• Breastfeeding
• Low amount of phosphate in the blood
• Increased blood acidity
• Overweight

Side Effects

• Minor effects like headache, diarrhea, nausea, weakness
• Potentially serious effects such as high cholesterol, kidney problems, depression, anxiety

Pregnancy & Lactation

• Pregnancy category B
• Use during pregnancy only if clearly needed
• CDC recommends HIV-1-infected mothers not breastfeed to avoid postnatal transmission

Precautions & Warnings

• Lactic acidosis and severe hepatomegaly with steatosis reported
• Exacerbation of hepatitis post treatment discontinuation
• Renal impairment possible
• Avoid certain coadministered products
• Risk of HIV-1 resistance in coinfected patients
• Fat redistribution observed in HIV-infected patients
• Immune reconstitution syndrome reported
