Indications
- Relief of symptoms associated with seasonal and perennial allergic rhinitis
- Relief of symptoms associated with chronic idiopathic urticaria
- Improves health-related quality of life and work/activity productivity
Pharmacology
- Works as an inverse agonist of the H1 receptor
- Prevents activation and downstream effects of histamine binding
- Potent and selective affinity for H1 receptors
- Does not carry antidopaminergic, antiserotonergic, anticholinergic, sedative, or adrenergic blocking activity
- Unlikely to cause significant CNS effects
Dosage & Administration
- Adults and children 12 years and older: 60 mg-180 mg once or twice daily
- Children 6 months to 11 years: 15 mg-60 mg once or twice daily
- Dosage adjustments for impaired renal function
Interaction
- No hepatic biotransformation, minimal interaction through hepatic mechanisms
- Coadministration with erythromycin or ketoconazole increases fexofenadine levels
- No interaction with omeprazole, reduced bioavailability with antacids
Contraindications
- Contraindicated in patients with known hypersensitivity to Fexofenadine Hydrochloride or any of its ingredients
Side Effects
- Nervous system disorders: headache, drowsiness, dizziness
- Gastrointestinal disorders: nausea
- Immune system disorders: hypersensitivity reactions
- Psychiatric disorders: insomnia, nervousness, sleep disorders
- Cardiac disorders: tachycardia, palpitations
- Skin and subcutaneous tissue disorders: rash, urticaria, pruritus
Pregnancy & Lactation
- Limited data on use during pregnancy, not recommended unless clearly necessary
- Not recommended for mothers breastfeeding their babies
Precautions & Warnings
- Limited data in older people, renally or hepatically impaired patients
- Patients with history of cardiovascular disease should be warned about potential adverse reactions
- Unlikely to produce significant effects on ability to drive or use machines
Use in Special Populations
- Pharmacokinetics altered in individuals with renal impairment
- No substantial effect on pharmacokinetics in moderate to severe hepatic disease
- Increased bioavailability in individuals over 65 years of age
Overdose Effects
- Dizziness, drowsiness, fatigue, and dry mouth reported with overdose
- No clinically significant adverse reactions with single doses up to 800 mg
- Symptomatic and supportive treatment recommended, haemodialysis ineffective in removing Telfex
