Description

  • Phenyltriazine antiepileptic
  • Treatment for some types of epilepsy and bipolar disorder
  • Controls epileptic seizures by inhibiting voltage-sensitive sodium channels
  • Stabilizes neuronal membranes to inhibit neurotransmitter release

Pharmacology

  • Inhibits voltage-sensitive sodium channels
  • Modulates release of presynaptic excitatory neurotransmitters
  • Binds weakly to various receptors such as serotonin, adenosine, dopamine, GABA, and histamine
  • Inhibits Cav2.3 (R-type) calcium currents

Dosage & Administration

  • Escalation regimen for patients with epilepsy aged 2 years and older
  • Escalation regimen for patients with bipolar disorder aged 18 years and older
  • Initial weight-based dosing guide for pediatric patients with epilepsy
  • Usual maintenance dose for different patient groups

Interaction

  • Impact of valproate, carbamazepine, phenytoin, phenobarbital, rifampin, estrogen-containing oral contraceptives, protease inhibitors on Lamitrin concentrations

Side Effects

  • Adult side effects: dizziness, headache, diplopia, nausea, blurred vision, rash
  • Children side effects: vomiting, diarrhea, infection, fever, abdominal pain, tremor

Pregnancy & Lactation

  • Pregnancy category C
  • Excretion in human milk
  • Consideration for nursing mothers

Precautions & Warnings

  • Discontinue at the first sign of rash
  • Monitoring for blood dyscrasias, suicidal behavior, ideation, and aseptic meningitis
