Description
Nexcal-D is a combination of calcium carbonate and vitamin D3 indicated for the treatment and prevention of osteoporosis, osteomalacia, tetany, hypoparathyroidism, and disorders of osteogenesis. It is also used as a supplement in case of inadequate intake of calcium in childhood diet, rickets, pregnancy and lactation, and in elderly patients. Nexcal-D helps prevent and treat calcium and vitamin D deficiency, and improves bone health in the elderly.

Composition
- Each tablet contains: Calcium Carbonate USP 1250 mg (Coral source), Vitamin D USP 200 IU
- Each DX tablet contains: Calcium Carbonate USP 1500 mg (Coral source), Vitamin D USP 400 IU
- Each effervescent tablet contains: Calcium Carbonate (Coral source) USP 1500 mg, Vitamin D3 400 IU as Cholecalciferol USP
Pharmacology
Calcium is vital for bodily functions and helps in the prevention and treatment of osteoporosis and associated fractures. Vitamin D aids in calcium absorption and stimulates bone formation. Calcium and vitamin D have synergistic effects on bone growth and in osteoporosis and fracture prevention.
Dosage and administration
One tablet once or twice daily with plenty of water, or as directed by the physician. Effervescent tablets should be dissolved in a glass of water and taken orally. Taking with a full stomach ensures better absorption.
Interaction
- Thiazide diuretics reduce urinary excretion of calcium
- Systemic corticosteroids reduce calcium absorption
- Simultaneous treatment with ion exchange resins or laxatives may reduce vitamin D absorption
- Calcium carbonate may interfere with the absorption of tetracycline preparations
- Hypercalcaemia may increase the toxicity of cardiac glycosides during treatment with calcium and vitamin D
- Nexcal-D should be administered at least three hours before the intake of certain medications
Contraindications
- Hypercalcemia and hyperparathyroidism
- Hypercalciuria and nephrolithiasis
- Hypersensitivity to the components of this preparation
- Severe renal insufficiencies
- Concomitant digoxin therapy

Pregnancy and lactation
- Daily intake should not exceed 1500 mg calcium and 600 IU cholecalciferol during pregnancy
- Caution should be taken to avoid overdoses as it may cause adverse effects on the developing fetus
- Vitamin D & Calcium pass into breast milk
- Additional vitamin D to the child should be considered carefully
Precautions and warnings
- Serum calcium levels should be followed during long-term treatment
- Monitoring is especially important in elderly patients on concomitant treatment with cardiac glycosides or diuretics
- Dose adjustment is necessary in case of hypercalcaemia or impaired renal function
- Caution should be taken in patients with impairment of renal function
- Patients with severe renal insufficiency may require alternative forms of vitamin D
- Caution should be taken in immobilized patients with osteoporosis
- Vitamin D dose should be considered when prescribing other drugs containing vitamin D
Overdose effects
- Hypervitaminosis D
- Hypercalcaemia
- Symptoms may include anorexia, nausea, vomiting, constipation, abdominal pain, muscle weakness, fatigue, mental disturbances, polydipsia, polyuria, bone pain, and in severe cases, cardiac arrhythmias
