Indications
- Active duodenal ulcer
- Active benign gastric ulcer
- Symptomatic erosive or ulcerative gastro-esophageal reflux disease (GERD)
- Gastro-esophageal Reflux Disease Long-term Management (GERD Maintenance)
- Symptomatic treatment of moderate to very severe gastro-esophageal reflux disease (symptomatic GERD)
- Zollinger-Ellison Syndrome
- In combination with appropriate antibacterial therapeutic regimens for the eradication of Helicobacter pylori in patients with peptic ulcer disease
Pharmacology
- Suppresses gastric acid secretion
- Inhibits the gastric H+/K+-ATPase
Dosage
- 20 mg for active duodenal ulcer and active benign gastric ulcer once daily in the morning
- 20 mg for erosive or ulcerative gastro-esophageal reflux disease once daily for 4-8 weeks
- Maintenance dose of 20 mg or 10 mg once daily for long-term management
Administration
- To be taken in the morning before eating
- Tablets should not be chewed or crushed, but swallowed whole
Interaction
- Potential interaction with pH-dependent absorption compounds
- Decrease in antifungal plasma levels when co-administered with ketoconazole or itraconazole
- No interaction with liquid antacids observed
- Absorption of atazanavir is pH-dependent, hence should not be co-administered with rabeprazole
Contraindications
- Hypersensitivity to the active substance or excipients
- Contra-indicated in pregnancy and during breastfeeding

Precautions & Warnings
- Symptomatic response to therapy does not preclude the presence of gastric or esophageal malignancy
- Modestly increased risk of hip, wrist and spine fracture in high doses and over long durations
- Risk of cross-hypersensitivity reactions
- Risk of blood dyscrasias, hepatic enzyme abnormalities, and hypomagnesaemia
- Reduced absorption of vitamin B12
- Association with subacute cutaneous lupus erythematosus (SCLE)
- Interference with laboratory tests
Use in Special Populations
- No dosage adjustment for patients with renal or hepatic impairment
- Not recommended for use in children due to lack of data on safety and efficacy
Overdose Effects
- Maximum established exposure is 60 mg twice daily or 160 mg once daily
- Effects are generally minimal and reversible without further medical intervention
- No specific antidote known
