Indications

  • Treatment of major depressive disorder (MDD)

Pharmacology

  • Enhances central noradrenergic and serotonergic activity
  • Acts as an antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
  • Antagonist of 5-HT<sub>2</sub> and 5-HT<sub>3</sub> receptors
  • Antagonist of histamine (H<sub>1</sub>) receptors
  • Moderate peripheral &alpha;1-adrenergic antagonist
  • Moderate antagonist at muscarinic receptors
  • Rapid and well-absorbed after oral administration
  • Peak plasma levels reached after about 2 hours
  • Approximately 85% binding to plasma proteins
  • Mean half-life of elimination is 20-40 hours; (26 hours in males, 37 hours in females)

Dosage & Administration

  • Recommended starting dose: 15 mg/day
  • Effective dose range: 15 to 45 mg/day
  • Recommended administration in the evening or prior to sleep
  • Not recommended for use in children
  • Dosage changes should be made at intervals of less than 1 to 2 weeks

Interaction

  • Clinically significant drug-drug interactions with MAOI & other serotonergic drugs
  • May interrupt the metabolism or activity of Carbamazepine, Phenytoin, or Cimetidine
  • Patients should avoid Alcohol & Diazepam

Contraindications

  • Contraindicated in known hypersensitivity to Mirtazapine or its excipients
  • Concomitant use with MAO inhibitors is contraindicated

Side Effects

  • Common side effects include dizziness, drowsiness, dry mouth, increased appetite, weight gain

Pregnancy & Lactation

  • Pregnancy Category-C
  • Patients should notify their physician if they become pregnant or intend to become pregnant during therapy
  • Patients should notify their physician if they are breastfeeding an infant

Precautions & Warnings

  • Patients, families, and caregivers should be alert to the emergence of certain behavioral changes
  • Patients should be warned about the risk of developing agranulocytosis
  • May impair judgment, thinking, and motor skills
  • Clinically significant ALT (SGPT) elevations (&ge;3 times the upper limit of the normal range) may occur
