Indications

  • Treatment of major depressive disorder (MDD)

Pharmacology

  • Enhances central noradrenergic and serotonergic activity
  • Antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
  • Potent antagonist of 5- HT<sub>2</sub> and 5-HT<sub>3</sub> receptors
  • Moderate peripheral &alpha;1-adrenergic antagonist
  • Moderate antagonist at muscarinic receptors
  • Potent antagonist of histamine (H<sub>1</sub>) receptors
  • Rapid and well-absorbed after oral administration
  • Peak plasma levels reached after about 2 hours
  • Mean half-life of elimination is 20-40 hours
  • Displays linear pharmacokinetics within the recommended dose range
  • Metabolized and eliminated via urine and faeces

Dosage & Administration

  • Recommended starting dose is 15 mg/day
  • Effective dose range is 15 to 45 mg/day
  • Half-life of elimination justifies once-a-day dosing
  • Dose changes should be made at intervals of less than 1 to 2 weeks
  • Not recommended for use in children

Interaction

  • Clinically significant drug-drug interactions with Monoamine Oxidase Inhibitors (MAOI) & other serotonergic drugs
  • May interrupt the metabolism or activity of Carbamazepine, Phenytoin or Cimetidine
  • Patient should avoid Alcohol & Diazepam

Contraindications

  • Contraindicated in patients with known hypersensitivity to Mirtazapine or its excipients
  • Concomitant use with a monoamine oxidase (MAO) inhibitor is contraindicated
  • Should not be used within 14 days of initiating or discontinuing therapy with a MAOI

  • Weight gain

Pregnancy & Lactation

  • Pregnancy Category-C
  • Patients should notify physician if they become pregnant or intend to become pregnant
  • Patients should notify physician if they are breastfeeding

Precautions & Warnings

  • Patients should be alert to the emergence of various behavioral changes
  • Warn about the risk of developing agranulocytosis
  • May impair judgment, thinking, and motor skills
  • Clinically significant ALT (SGPT) elevations may occur
