Indications
- Adjunct treatment of parkinsonism
- Useful as adjuvant therapy with levodopa
- Control of extrapyramidal disorders
Description
- Exerts direct inhibitory effect on parasympathetic nervous system
- Has relaxing effect on smooth musculature
Pharmacology
- Non-selective muscarinic acetylcholine receptor antagonist
- Binds with higher affinity to M1 subtype
- Higher affinity for central muscarinic receptors in cerebral cortex
- Lower affinity for peripheral muscarinic receptors
- May modify nicotinic acetylcholine receptor neurotransmission
- May lead indirectly to enhanced dopamine release in striatum
Dosage & Administration
- Individualized dosage
- Low initial dose, increased gradually
- Initial therapy for parkinsonism with 1 mg Trihexyphenidyl
- Total daily dose usually 6 to 10 mg, some patients may require 12 to 15 mg
- Control of extrapyramidal reactions to tranquilizers with 1 to 15 mg dosage
- Concomitant use with levodopa requires careful adjustment of each dosage
- Total daily intake divided into 3 doses, or 4 doses for high doses (>10 mg daily)
- Safety and effectiveness in pediatric patients not established
Interaction
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with alcohol or CNS depressants
- Monoamine oxidase inhibitors and tricyclic antidepressants intensify anticholinergic effects
- Increased risk of tardive dyskinesia with anticholinergics and neuroleptics
- Possible need to reduce usual dose of either Trihexyphenidyl or levodopa during concomitant therapy
Contraindications
- Hypersensitivity to Trihexyphenidyl
- Narrow angle glaucoma
Side Effects
- Dryness of the mouth
- Blurred vision
- Dizziness
- Mild nausea
- Nervousness
- Suppurative parotitis
- Skin rashes
- Dilatation of the colon
- Paralytic ileus
- Psychiatric manifestations such as delusions, hallucinations, paranoia
- Cognitive dysfunctions, including confusion and memory impairment
- Constipation
- Drowsiness
- Urinary hesitancy or retention
- Tachycardia
- Dilation of the pupil
- Increased intraocular pressure
- Choreiform movements
- Weakness
- Vomiting
- Headache
- Exacerbation of parkinsonism with abrupt treatment withdrawal
- Neuroleptic malignant syndrome with abrupt treatment withdrawal
Pregnancy & Lactation
- No controlled data in animal or human pregnancy
- Use during pregnancy only if benefit outweighs risk
- Caution with nursing women, may cause suppression of lactation
Precautions & Warnings
- Gonioscope evaluation prior to initiation of therapy
- Close monitoring of intraocular pressures
- Possibility of angle closure with increase in intraocular pressure
- Use with caution in hot weather and with other atropine-like drugs
- Anhidrosis may occur more readily in presence of existing sweat disturbance
- Consideration of hyperthermia if evidence of anhidrosis
Overdose Effects
- Reported doses up to 300 mg ingested without fatalities or sequelae
- Rare cases of death associated with overdoses taken with other CNS-depressant agents
- Typical central symptoms of atropine intoxication
- Neuropsychiatric signs such as delirium, seizures may be present
- Treatment involves symptomatic and supportive therapy
- Gastric lavage or methods to limit absorption
- Respiratory support, fluid volume replacement, acid-balance maintenance
