  • May lead indirectly to enhanced dopamine release in striatum

Dosage & Administration

  • Individualized dosage
  • Low initial dose, increased gradually
  • Initial therapy for parkinsonism with 1 mg Trihexyphenidyl
  • Total daily dose usually 6 to 10 mg, some patients may require 12 to 15 mg
  • Control of extrapyramidal reactions to tranquilizers with 1 to 15 mg dosage
  • Concomitant use with levodopa requires careful adjustment of each dosage
  • Total daily intake divided into 3 doses, or 4 doses for high doses (>10 mg daily)
  • Safety and effectiveness in pediatric patients not established

Interaction

  • Additive effects with cannabinoids, barbiturates, opiates, and alcohol
  • Increased sedative effects with alcohol or CNS depressants
  • Monoamine oxidase inhibitors and tricyclic antidepressants intensify anticholinergic effects
  • Increased risk of tardive dyskinesia with anticholinergics and neuroleptics
  • Possible need to reduce usual dose of either Trihexyphenidyl or levodopa during concomitant therapy

Contraindications

  • Hypersensitivity to Trihexyphenidyl
  • Narrow angle glaucoma

  • Possibility of angle closure with increase in intraocular pressure
  • Use with caution in hot weather and with other atropine-like drugs
  • Anhidrosis may occur more readily in presence of existing sweat disturbance
  • Consideration of hyperthermia if evidence of anhidrosis
