Indications

Description

  • Direct inhibitory effect on parasympathetic nervous system

Pharmacology

  • Muscarinic acetylcholine receptor antagonist
  • Higher affinity for central muscarinic receptors
  • Lower affinity for peripheral muscarinic receptors
  • Modifies nicotinic acetylcholine receptor neurotransmission
  • Enhanced dopamine release in striatum

Dosage & Administration

  • Individualized dosage
  • Initial low dose with gradual increase
  • Adjustment based on patient reaction
  • Preferable administration before or after meals
  • Dose management for different forms of parkinsonism
  • Dose adjustment in concomitant use with levodopa
  • Optimal dosing for children not established

Interaction

  • Additive effects with cannabinoids, barbiturates, opiates, alcohol
  • Increased sedative effects with alcohol or CNS depressants
  • Intensification of anticholinergic effects with monoamine oxidase inhibitors, tricyclic antidepressants
  • Increased risk for tardive dyskinesia with concurrent administration of anticholinergics and neuroleptics

Contraindications

  • Hypersensitivity to trihexyphenidyl
  • Narrow angle glaucoma

Precautions & Warnings

  • Gonioscope evaluation prior to therapy
  • Close monitoring of intraocular pressures
  • Possible angle closure and increase in intraocular pressure
  • Caution in hot weather, alcoholics, those with CNS disease, and manual laborers in hot environments
  • Potential for anhidrosis and hyperthermia
  • Possible occurrence of Neuroleptic Malignant Syndrome

Overdose Effects

  • Central anticholinergic syndrome
  • Dilated and sluggish pupils
  • Warm, dry skin
  • Facial flushing
  • Decreased secretions
  • Foul-smelling breath
  • Elevated temperature
  • Tachycardia
  • Cardiac arrhythmias
  • Decreased bowel sounds
  • Urinary retention
  • Neuropsychiatric signs
  • Stupor
  • Coma
  • Paralysis
  • Cardiac and respiratory arrest
  • Symptomatic and supportive therapy for treatment
  • Gastric lavage or other methods to limit absorption
  • Administration of diazepam or a short-acting barbiturate if CNS excitation is observed
  • Contraindication of phenothiazines
  • Respiratory support and vasopressor agents may be necessary
  • Reversal of hyperpyrexia, fluid volume replacement, and acid-balance maintenance
  • Unknown dialyzability
