Indications
- Hypertension management
- Treatment of angina pectoris

Pharmacology
- Selective beta1-blocker
- Reduction of heart rate
- Inhibition of catecholamines
- Reduction of cardiac output
- Interference with insulin release
- Interference with carbohydrate metabolism
Dosage & Administration
- Total daily dosage range: 100-400 mg
- Initial dose: 100 mg/day
- Dose escalation at weekly intervals
- Twice or thrice daily dosing for angina
- Control of cardiac arrhythmias with 50-300 mg/day
- Four times daily dosing for hyperthyroidism
- Initial oral dose for myocardial infarction: 50 mg every 6 hours for 48 hours
Interaction
- Additive effect with catecholamine-depleting drugs
- Increased concentration with CYP2D6 inhibitors
- Risk of bradycardia with concomitant digitalis glycosides
- Exacerbation of rebound hypertension with clonidine withdrawal
Contraindications
- AV block
- Uncontrolled heart failure
- Severe bradycardia
- Sick-sinus syndrome
- Cardiogenic shock
- Severe peripheral arterial disease
- Known hypersensitivity to Metoprolol or other B-blockers
- Complicated myocardial infarction with significant bradycardia
- First-degree heart block
- Systolic hypotension
- Severe heart failure

- Use with caution in patients with bronchospastic disease

- Masking of tachycardia occurring with hypoglycemia
- Caution in patients with peripheral vascular disease
- Caution when used with calcium channel blockers
- Use with caution in patients with impaired hepatic function
- No clinically relevant differences in the adverse event profile for pediatric patients aged 6 to 16 years
