Dobumin 12.5 mg/ml (IV Injection)

Indications

• Treatment of patients with hypoperfusion states
• Inotropic support when cardiac output is insufficient
• Inotropic support for patients with increased ventricular filling pressures
• Treatment of pulmonary congestion and oedema risk

Pharmacology

• Positive inotropic effect
• Stimulation of β1-adrenergic receptors
• Increase in myocardial contractility
• Increase in stroke volume
• Increase in cardiac output

Dosage

• Recommended dosage range of 2.5 to 40 mcg/kg/min
• Adjustment of rate and duration based on patient's response
• Infusion based on concentration of Dobutamine Hydrochloride
• Final volume determined by patient's fluid requirements
• Gradual decrease in dosage upon discontinuation

Administration

• Continuous intravenous infusion
• Steady-state dobutamine plasma concentration achieved within 10 minutes
• No loading doses or bolus injections required

Interaction

• Potency decrease with β-adrenergic receptor antagonists
• Unopposed α-agonist effects in case of antagonists
• No overt indications of medicine interactions in clinical studies

Contraindications

• Previous hypersensitivity to Dobutamine Hydrochloride

Side Effects

• Increased heart rate and blood pressure
• Ventricular ectopic activity
• Hypotension
• Reactions at sites of intravenous infusion
• Miscellaneous uncommon effects like nausea, headache, palpitations
• Mild reduction in serum potassium concentration
• No adverse effects with infusions up to 72 hours

Pregnancy & Lactation

• Not recommended during pregnancy unless benefits outweigh risks
• Caution during nursing, breastfeeding should be discontinued during treatment

Precautions & Warnings

• Close monitoring of heart rate, rhythm, and arterial blood pressure
• Electrocardiographic monitoring during therapy initiation

Overdose Effects

• Toxicity symptoms including anorexia, nausea, vomiting, tremor
• Positive inotropic and chronotropic effects leading to hypertension
• Treatment involves discontinuing administration, establishing airway, ensuring oxygenation and ventilation
• Severe ventricular tachyarrhythmias may be treated with propranolol or lidocaine
• Hypertension responds to reduction in dose or discontinuation of therapy
