Indications
- Treatment of hypoperfusion states

Pharmacology
- Positive inotropic effect
- Stimulation of beta-1 adrenergic receptors
- Increase in myocardial contractility

Dosage
- Recommended infusion rate range of 2.5 to 40 mcg/kg/min
- Adjustment of rate and duration based on patient's response

Administration
- Continuous intravenous infusion
- Achievement of steady-state dobutamine plasma concentration within 10 minutes
- Avoidance of loading doses or bolus injections
Interaction
- Decreased potency when given with beta-adrenergic receptor antagonists
- Possible apparent effects due to alpha-adrenergic blockade
- No overt indication of interactions with other medications
Contraindications
- Hypersensitivity to Dobutamine Hydrochloride
Side Effects
- Increased heart rate and blood pressure
- Occasional premature ventricular beats
- Hypotension
- Phlebitis and local inflammatory changes
- Nausea, headache, chest pain, palpitations, and shortness of breath
- Isolated cases of thrombocytopenia
- Mild reduction in serum potassium concentration
- No adverse effects seen with infusions up to 72 hours
Pregnancy & Lactation
- Not recommended during pregnancy unless benefits outweigh risks
- Caution advised during lactation
Precautions & Warnings
- Close monitoring of heart rate, blood pressure, and infusion rate
- Electrocardiographic monitoring upon therapy initiation
Overdose Effects
- Toxicity symptoms include anorexia, nausea, vomiting, tremor, anxiety, palpitations, headache, and more
- Possible complications such as hypertension, tachyarrhythmias, myocardial ischemia, and ventricular fibrillation
- Treatment involving discontinuation, airway establishment, resuscitative measures, and monitoring
- Use of propranolol or lidocaine for severe arrhythmias
- No established benefit from forced diuresis, peritoneal dialysis, hemodialysis, or charcoal hemoperfusion
