Indications
- For correction of haemodynamic imbalance in acute hypotension or shock
- Adjunct after open heart surgery for persistent hypotension
- Use in chronic cardiac decompensation as in congestive failure
Pharmacology
- Stimulates alpha, beta, and dopamine receptors
- Functions as a natural catecholamine
- Produces positive chronotropic and inotropic effects on the myocardium
Dosage
- Adult dose of 2 to 5 micrograms/kg/min for modest increment of heart force and renal perfusion
- Gradual increase to 20 to 50 micrograms/kg/min in seriously ill patients if needed
- Start dose of 0.5 to 2 micrograms/kg/min for refractory chronic congestive heart failure
Administration
- Administered intravenously through a suitable intravenous catheter or needle
- Controlled using an intravenous drip chamber or metering device
- Individual titration for desired haemodynamic and/or renal response
Interaction
- Potentiated by monoamine oxidase inhibitors (MAOI's)
- Antagonized by beta-adrenergic blocking agents like Propranolol
- May increase the effect of diuretic agents
Contraindications
- Hypersensitivity to dopamine or any of its ingredients
- Phaeochromocytoma, uncorrected tachyarrhythmias, or ventricular fibrillation
Side Effects
- Ectopic beats, nausea, vomiting, tachycardia
- Anginal pain, palpitations, dyspnoea, headache
- Hypotension, hypertension, vasoconstriction
Pregnancy & Lactation
- No evidence of teratogenic effects in animal studies
- Use in pregnant women if benefits outweigh potential risks to the fetus
- Not recommended for breastfeeding mothers unless expected benefits outweigh risks
Precautions & Warnings
- Should not be administered in the presence of uncorrected tachyarrhythmia or ventricular fibrillation
- Metabolized by monoamine oxidase (MAO) and catechol-O-methyltransferase (COMT)
- Close monitoring advised in patients with impaired renal and hepatic function
Overdose Effects
- Reduce rate of administration in case of excessive blood pressure elevation
- Use of short-acting alpha-adrenergic blocking agent such as Phentolamine may be considered
Therapeutic Class
- Inotropic-sympathomimetics
Reconstitution
- Must be diluted before administration to patients
- Stable for a minimum of 24 hours after dilution in specified sterile intravenous solutions
- Do not add Cardopa solution to alkaline intravenous solutions
