Indications
- Recommended for correction of haemodynamic imbalance
- Used for acute hypotension or shock
- Adjunct after open heart surgery

Pharmacology
- Stimulates alpha, beta, and dopamine receptors
- Natural catecholamine
- Produces positive chronotropic and inotropic effects
- Does not cross the blood-brain barrier
Dosage
- Adult dosage range: 2 to 50 micrograms/kg/min
- Gradual dosage increase for seriously ill patients
- Specific dosage for chronic congestive heart failure
- Monitoring of ECG, blood pressure, and urine output
Administration
- Administered intravenously
- Controlled rate of flow in drops per minute
- Individual titration for desired haemodynamic response
- Adjustment of drug concentration to avoid unnecessary fluid expansion
Interaction
- Potentiated by monoamine oxidase inhibitors
- Antagonized by beta-adrenergic blocking agents
- Possible effects with anesthetics, ergot alkaloids, antidepressants, and diuretic agents
- Use of vasodilators in patients with heart failure
Contraindications
- Hypersensitivity to dopamine or its ingredients
- Phaeochromocytoma
- Uncorrected tachyarrhythmias or ventricular fibrillation

Pregnancy & Lactation
- No evidence of teratogenic effects in animal studies
- Use in pregnant women when benefits outweigh risks
- Unknown excretion in breast milk
Precautions & Warnings
- Not to be administered with uncorrected tachyarrhythmia or ventricular fibrillation
- Metabolized by monoamine oxidase and catechol-O-methyltransferase
- Close monitoring in patients with impaired renal and hepatic function
- Careful correction of hypovolaemia prior to treatment
Overdose Effects
- Excessive blood pressure elevation
- Temporary discontinuation until stabilization
- Consideration of short-acting alpha-adrenergic blocking agent
Therapeutic Class

Reconstitution
- Must be diluted before administration
- Stable for a minimum of 24 hours after dilution
- Avoid adding to alkaline intravenous solutions
- Avoid mixing with alteplase or amphotericin B solution
