Indications
- Bronchial asthma
- Bronchospasm
- Chronic obstructive pulmonary disease (COPD)
- Pulmonary disease with spastic bronchial component
Pharmacology
- Doxophylline is a novel bronchodilator
- Structurally differs from Theophylline
- Selective inhibition of phosphodiesterase 4
- Relaxes bronchial smooth muscle
- Decreased affinities toward adenosine A1 and A2 receptors
- Inhibition of platelet activating factor (PAF) and generation of leukotriene production
Dosage and administration
- Elderly: 200 mg tablet two or three times daily
- Adults: 400 mg tablet two or three times daily or as prescribed by the physician
- Children (>12 years of age): 10 ml syrup or 200 mg tablet two or three times daily
- Children (6-12 years of age): 6-9 mg/kg body weight two times daily
- Doxophylline SR tablet to be taken once daily if required daily dose is 400 mg
Interaction
- Should not be administered with other xanthine preparations
- Limit consumption of beverages and food containing caffeine
- Caution with ephedrine or other sympathomimetic drugs
- Concurrent administration with certain drugs may reduce hepatic clearance of xanthine derivatives or increase clearance
- Phenytoin, other anticonvulsants, and cigarette smoking may influence clearance of xanthine derivatives
Contraindications
- Known hypersensitivity to the drug or other xanthine derivatives
- Acute myocardial infarction
- Hypotension
- During lactation
Side effects
- Nausea
- Vomiting
- Epigastric pain
- Headache
- Irritability
- Insomnia
- Tachycardia
- Extrasystoles
- Tachypnea
- Rare cases of hyperglycemia or albuminuria
- Severe cardiac arrhythmias and tonic-clonic seizure in case of overdose
Pregnancy and lactation
- Animal tests have shown no interference with pre and postnatal growth
- Insufficient clinical evidence about effects during pregnancy, use should be evaluated carefully
- Contraindicated during lactation
- Does not affect ability to drive and use machines
Precautions and warnings
- Factors may reduce hepatic clearance of xanthine derivatives and increase plasma levels
- Monitoring of blood levels recommended in certain cases
- Caution with patients having cardiac disease, hypertension, severe hypoxemia, hyperthyroidism, liver disease, renal impairment
- Particular caution in patients with congestive heart failure
Overdose effects
- Severe cardiac arrhythmias and tonic-clonic seizure may occur
- Discontinuation of treatment may be necessary in case of overdose
- Symptomatic treatment of cardiovascular collapse should be instituted
- No specific antidote available
