Indications
- Adjunct to diet and exercises to improve glycaemic control in type 2 diabetes mellitus
- Combination therapy for patients not adequately controlled on Metformin or Vildagliptin alone
Pharmacology
- Vildagliptin inhibits DPP-4 enzyme
- Vildagliptin increases insulin secretion and decreases glucagon secretion
- Metformin lowers both basal and postprandial plasma glucose
Dosage & Administration
- Twice daily dosing for adults
- Initiate based on patient's current Metformin dose
- May be taken with or just after food to reduce gastrointestinal symptoms
Interaction
- No clinically relevant pharmacokinetic interaction observed with Vildagliptin and Metformin
- Vildagliptin has low potential for drug interactions
Contraindications
- Hypersensitivity to Vildagliptin or Metformin
- Renal disease or dysfunction
- Acute myocardial infarction
- Congestive heart failure
- Metabolic acidosis including diabetic ketoacidosis

- Nausea
- Hypoglycemia
Pregnancy & Lactation
- Not recommended during pregnancy
- Should not be administered to breastfeeding women
Precautions & Warnings
- Lactic acidosis risk with Metformin accumulation
- Monitoring of serum creatinine and liver function tests recommended
- Use with caution in elderly patients and those with renal impairment or hepatic impairment
Use in Special Populations
- Not recommended for pediatric patients below 18 years
- Monitoring of renal function in geriatric patients
- Contraindicated in patients with renal or hepatic impairment
