Description
- Primace is indicated for the treatment of hypertension, congestive heart failure, post-myocardial infarction complications, and reduction of cardiovascular risk.
- It is an angiotensin-converting enzyme (ACE) inhibitor that blocks the conversion of angiotensin I to angiotensin II, leading to decreased vasopressor activity and aldosterone secretion.
- Dosage must be adjusted based on the patient's condition. Recommended dose for hypertension is 1.25-2.5 mg once daily, for heart failure after myocardial infarction it's 2.5 mg twice daily, and for prevention of major cardiovascular events it's 2.5 mg once daily for the first week, followed by 5 mg once daily.
- Primace tablets should be swallowed with sufficient liquid and must not be chewed or crushed. It can be taken before, during, or after a meal.
- Contraindications include hypersensitivity to ramipril, history of angioedema, concomitant use with sacubitril/valsartan therapy, and pregnancy.
- Side effects may include dizziness, headache, fatigue, asthenia, hypotension, cough, nausea, vomiting, rash, urticaria, and hyperkalaemia.
- It is not recommended during pregnancy or breastfeeding, and precautions should be taken in patients with impaired renal function, hyperkalaemia, hypotension, and impaired hepatic function.
- It should be used with caution in special populations such as the elderly, patients with hepatic or renal impairment, and those pretreated with diuretics.
- Overdosage may cause excessive peripheral vasodilatation, bradycardia, electrolyte disturbances, and renal failure. Management involves primary detoxification and administration of adrenergic agonists if necessary.
