Indications
- Treatment of hypertension
- Management of congestive heart failure
- Reduction of risk of stroke, myocardial infarction, and cardiovascular death
Pharmacology
- ACE inhibitor
- Blocks conversion of angiotensin I to angiotensin II
- Decreases vasopressor activity and aldosterone secretion
Dosage
- Initial dose for hypertension: 1.25-2.5 mg once daily
- Maintenance dosage for hypertension: 2.5-20 mg daily
- Initial dose for congestive heart failure after myocardial infarction: 2.5 mg twice daily
- Titration to a target daily dose of 5 mg twice daily for heart failure
- Recommended dose for prevention of major cardiovascular events: 2.5 mg once daily for the first week, 5 mg once daily for the following 3 weeks
Administration
- Tablets to be swallowed with sufficient liquid
- Absorption not significantly affected by food
Interaction
- May lead to serious hypotension when administered with diuretics
- Increased serum lithium concentration with lithium therapy
- Reduction in blood pressure may affect driving and operating machinery, exacerbated by alcohol
- NSAIDs may reduce antihypertensive effect and cause deterioration of renal function
Contraindications
- Hypersensitivity to ramipril or any other ACE inhibitor
- History of angioedema
- Concomitant use with sacubitril/valsartan therapy
- Haemodynamically relevant renal artery stenosis
- Hypotensive or haemodynamically unstable states
- Concomitant use with aliskiren-containing medicines in diabetes or moderate to severe renal impairment
- Concomitant use with angiotensin II receptor antagonists (AIIRAs) in diabetic nephropathy
- During pregnancy

Pregnancy & Lactation
- Must be avoided during pregnancy
- Pregnancy must be excluded before starting treatment
- Not recommended during breastfeeding
Precautions & Warnings
- Should be used with caution in patients with impaired renal function, hyperkalaemia, hypotension, and impaired hepatic function
Use in Special Populations
- Elderly: Consider reduced initial dose
- Hepatic impairment: Initiate treatment under close medical supervision
- Renal impairment: Adjust dosage according to creatinine clearance
- Patients pretreated with a diuretic: Consider discontinuation or dose reduction of the diuretic before starting treatment
Overdose Effects
- Signs and symptoms: Excessive peripheral vasodilatation, electrolyte disturbances, renal failure
- Management: Primary detoxifcation, administration of α1-adrenergic agonists, volume and salt substitution
