Indications
- Hypertension

Pharmacology
- Angiotensin converting enzyme (ACE) inhibitor
- Blocks the conversion of angiotensin I to angiotensin II
- Decreased vasopressor activity
- Decreased aldosterone secretion
- Antihypertensive activity
- Management of heart failure
- Reduction of stroke and myocardial infarction risk
- Long acting and well tolerated
Dosage
- Adjustment according to patient tolerance and response
- Usual initial dose of 1.25-2.5 mg for hypertension
- Titration to target daily dose for heart failure after myocardial infarction
- Initial dose for prevention of major cardiovascular events
- Dosage adjustments in renal impairment
Administration
- Tablets swallowed with liquid
- Not to be chewed or crushed
- Absorption not significantly affected by food
Interaction
- Concomitant administration with diuretics
- Concomitant therapy with lithium
- Reduction in BP affecting ability to drive and operate machinery
- NSAIDs reducing antihypertensive effect
Contraindications
- Hypersensitivity to ramipril or any other ACE inhibitor
- History of angioedema
- Concomitant use with sacubitril/valsartan therapy
- Haemodynamically relevant renal artery stenosis
- Hypotensive or haemodynamically unstable states
- Concomitant use with aliskiren-containing medicines in patients with diabetes or moderate to severe renal impairment
- Concomitant use with angiotensin II receptor antagonists in patients with diabetic nephropathy
- During pregnancy

- Use with caution in impaired renal function
- Use with caution in hyperkalaemia
- Use with caution in hypotension
- Use with caution in impaired hepatic function
Use in Special Populations
- Reduced initial dose for elderly patients
- Close medical supervision for hepatic impairment
- Adjustment of initial dose for renal impairment
- Consideration for patients pretreated with a diuretic
Overdose Effects
- Excessive peripheral vasodilatation
- Marked hypotension
- Shock
- Bradycardia
- Electrolyte disturbances
- Renal failure
