Indications

• Prophylactic treatment of vascular headache of migraine type
• Classical migraine
• Common migraine
• Cluster headache

Description

• Tricyclic compound
• Antagonizes serotonin receptor competitively
• Antihistamine properties
• Antibradykinin properties
• Weak anticholinergic properties
• Appetite-stimulating properties

Pharmacology

• Inhibits peripheral actions of serotonin and histamine
• Increases membrane permeability of cranial vessels
• Alters pain thresholds in migraines
• Blocks 5-HT receptors
• Attenuates signaling of serotonin in causing cranial vasoconstriction
• Inhibits peripheral actions of bradykinin
• May inhibit serotonin reuptake by blood platelets
• Decreases passive distension of extracranial arteries

Dosage & Administration

• Adults: Usually 1.5 mg daily
• May be taken as a single dose at night or in three divided doses
• Dosage should be adjusted to individual patient requirements
• Maximum of 4.5 mg daily
• Up to 3 mg may be given as a single dose
• Children (aged over 2 years): Up to 1.5 mg daily
• Up to 1 mg as a single dose at night

Interaction

• Enhances central effects of sedatives, hypnotics, antihistamines, and alcohol
• Antagonizes the hypotensive effect of adrenergic neurone blockers

Contraindications

• Known hypersensitivity to any of its ingredients

Side Effects

• Appetite stimulating effect
• Increase in body weight
• Drowsiness

Pregnancy & Lactation

• Limited information of using Pizotifen in pregnancy
• No ill effects reported
• Should only be given during pregnancy if the potential benefit to mother justifies the potential risk to the fetus
• Safety during lactation not established

• Urinary retention
• Renal impairment
• Drowsiness affecting driving & operation of machineries
• Enhances the action of alcohol
